Utility and Applicability of Rapid Diagnostic Testing in Antimicrobial Stewardship in the Asia-Pacific Region: A Delphi Consensus

Anucha Apisarnthanarak et al.

Summary: A group of experienced practitioners in the Asia-Pacific region have provided 10 consensus statements for the use of rapid diagnostic tests in antimicrobial stewardship. They offer guidance on definitions, types, deployment, measuring effectiveness, and overcoming challenges associated with the use of these tests. The study highlights the need for increased utilization of rapid diagnostic tests in the region to optimize antimicrobial stewardship.

BSAC Vanguard Series: Inequality and antibiotic resistance

Marc Mendelson

Summary: COVID-19 has highlighted the global inequities in access to tools needed for addressing a pandemic. Similarly, antibiotic resistance (ABR) is expected to cause even greater devastation. Tackling infectious diseases burden in low- and middle-income countries (LMICs) is crucial for impacting ABR worldwide. However, efforts to address antibiotic conservation have largely been unsuccessful, with insufficient resources dedicated to interventions such as infection prevention, sanitation, clean water, vaccination, and changes in agricultural practice. Large-scale public health interventions and funding mechanisms are essential, and action at the highest levels is required.

Global burden of bacterial antimicrobial resistance in 2019: a systematic analysis

Christopher J. L. Murray et al.

Summary: Antimicrobial resistance (AMR) poses a significant threat to global human health, and this study provides the most comprehensive estimates of AMR burden to date. By estimating deaths and disability-adjusted life-years (DALYs) associated with bacterial AMR in 204 countries and territories in 2019, the study highlights the impact of resistance and the leading pathogen-drug combinations contributing to it.

A review of national action plans on antimicrobial resistance: strengths and weaknesses

Angela Willemsen et al.

Summary: This study analyzed national action plans from the WHO Library to assess their alignment with the Global Action Plan on Antimicrobial Resistance. The findings revealed significant differences in the content of information provided by different countries, despite the availability of guidelines to inform the development of these plans.

Unravelling 'low-resource settings': a systematic scoping review with qualitative content analysis

Chanel van Zyl et al.

Summary: The effects of healthcare-related inequalities are most evident in low-resource settings, which are often not explicitly defined. A systematic scoping review found that major themes in the field of rehabilitation in low-resource settings include financial pressure, suboptimal healthcare service delivery, underdeveloped infrastructure, paucity of knowledge, research challenges, restricted social resources, geographical and environmental factors, human resource limitations, and the influence of beliefs and practices. These themes can help in understanding and promoting effective knowledge transfer in health-related research.

Shortage of essential antimicrobials: a major challenge to global health security

Nusrat Shafiq et al.

Summary: The lack of access to safe and effective antimicrobials is a threat to global health security and contributes to the emergence and spread of antimicrobial resistance. It is essential to conduct robust studies and involve government intervention to address this issue effectively.

Rapid diagnostic testing for antimicrobial stewardship: Utility in Asia Pacific

Anucha Apisarnthanarak et al.

Summary: Rapid diagnostic testing (RDT) is important for accurate identification of infectious organisms and antimicrobial stewardship programs, but its use in Asia Pacific is less common due to cost concerns. More data is needed on RDT use in AMS to assess impact on antimicrobial usage and patient outcomes. Regional consensus statements are needed to guide clinical practice and define a relevant role for RDT in infection management.

Patient understanding of and participation in infection-related care across surgical pathways: a scoping review

Oluchi Mbamalu et al.

Summary: A scoping review found a gap in patient understanding and participation in infection-related care in surgical specialties, highlighting the challenge of standardizing patient engagement strategies. The need for more inclusive and contextually fit patient engagement and participation strategies in infection-related care in surgery was emphasized.

Impact of the COVID-19 pandemic on the surveillance, prevention and control of antimicrobial resistance: a global survey

Sara Tomczyk et al.

Summary: The survey on the global impact of COVID-19 on AMR among GLASS countries revealed reduced availability of healthcare and cleaning staff, which could bias AMR surveillance data. Additionally, most countries still lack complete data to measure the specific impact of COVID-19 on AMR rates.

Navigating sociocultural disparities in relation to infection and antibiotic resistance-the need for an intersectional approach

Esmita Charani et al.

Summary: One of the main drivers of antibiotic resistance and drug-resistant bacterial infections is the overuse and misuse of antibiotics in human populations. Infection management and antibiotic decision-making are complex processes influenced by multiple factors such as social constructs, race, ethnicity, gender identity, cultural practices, migration status, and geography. These external drivers impact health outcomes at both individual and population levels, compromising healthcare services already burdened with drug-resistant infections.
