Prevalence and Significance of Impaired Microvascular Tissue Reperfusion Despite Macrovascular Angiographic Reperfusion (No-Reflow)

Felix C. Ng et al.

Summary: This study investigates the prevalence and clinical-radiologic features of no-reflow in human stroke. It finds that no-reflow is common and can be detected through perfusion imaging. The presence of no-reflow is associated with posttreatment complications and poor functional outcomes.

Dynamics of cerebral perfusion and oxygenation parameters following endovascular treatment of acute ischemic stroke

Gianluca Brugnara et al.

Summary: This study investigated the effects of endovascular treatment on cerebral perfusion and oxygenation parameters in patients with acute ischemic stroke and large vessel occlusion, showing that complete recanalization led to better perfusion and oxygenation compared to incomplete recanalization. Additionally, a reduction in the ischemic penumbra was associated with higher recanalization rates, highlighting the importance of targeting the penumbra for successful treatment outcomes.

Integrative cerebral blood flow regulation in ischemic stroke

Jui-Lin Fan et al.

Summary: Optimizing cerebral perfusion is crucial for rescuing salvageable ischemic brain tissue, yet there are no effective guidelines for blood pressure management in acute stroke. The control of cerebral blood flow involves complex pathways that are often overlooked in stroke management. The unique anatomy and physiology of cerebrovascular circulation are usually treated as a separate system rather than an integral part of the cardiovascular system.

Summary: This study describes different patterns of perfusion abnormalities observed within the first 24 hours after thrombolysis and compares characteristics of the patients according to their perfusion pattern. Hypoperfusion in the ischemic lesion and the surrounding tissue is associated with poor outcome.

Dynamic capillary stalls in reperfused ischemic penumbra contribute to injury: A hyperacute role for neutrophils in persistent traffic jams

Sefik E. Erdener et al.

Summary: Despite recanalization in acute ischemic stroke, less favorable outcomes may be attributed to the pathological phenomenon of dynamic flow stalls in cerebral capillaries. Modulation of these stalls can improve penumbral blood flow, oxygenation, reduce cellular damage, and improve functional outcomes after reperfusion. This dynamic microcirculatory stall phenomenon may contribute to ongoing penumbral injury and act as a potential hyperacute mechanism in ischemic stroke.

High-altitude rodents have abundant collaterals that protect against tissue injury after cerebral, coronary and peripheral artery occlusion

Summary: High-altitude species such as guinea pigs and deer mice have more collaterals, providing greater protection against stroke and lower levels of ischemia in the heart and extremities. The vessels of the circle of Willis in high-altitude species exhibit unique features.

Summary: Hyperperfusion on ASL after mechanical thrombectomy is associated with successful recanalization and improvement in NIHSS24h scores. In predicting good functional outcomes at 90 days, hyperperfusion, NIHSS24h scores, and lesion volume on diffusion-weighted imaging are independent variables.

Summary: The systematic review and meta-analysis confirmed the association between BBB disruption and hemorrhagic transformation in patients with acute ischemic stroke. Summary: This study found that favorable tissue-level collaterals predict smaller ischemic lesion Net Water Uptake after treatment in patients with acute ischemic stroke due to large vessel occlusion. Favorable TLC and decreased NWU were independent predictors of good functional outcomes in these patients.
