I've been to more of my friends' funerals than I've been to my friends' weddings: Witnessing and responding to overdose in rural Northern New England

Kerry Nolte et al.

Summary: Witnessing overdose is common among PWUD in Northern New England, causing trauma. PWUD are willing to respond to overdoses, but face barriers such as limited naloxone access and legal consequences. Providing effective overdose response tools and policies to protect PWUD can reduce overdose mortality.

The Impact of the COVID-19 Pandemic on Drug Use Behaviors, Fentanyl Exposure, and Harm Reduction Service Support among People Who Use Drugs in Rural Settings

Rebecca S. Bolinski et al.

Summary: The COVID-19 pandemic has exacerbated the opioid overdose crisis in the US, particularly in rural areas. Our study found that drug availability changed, overdose risk increased, and other drug-related harms were faced by people who use drugs in rural areas during the pandemic. It is important to ensure access to harm reduction services, including overdose prevention and drug checking for this vulnerable population.

PrEP Care Continuum Engagement Among Persons Who Inject Drugs: Rural and Urban Differences in Stigma and Social Infrastructure

Suzan M. Walters et al.

Summary: The study found that stigma is a major barrier to PrEP care engagement among people who inject drugs. Awareness and knowledge levels differ between urban and rural environments, but willingness to use PrEP is similar. Stigmas against drug use, HIV, and sexualities were identified as the main barriers to PrEP uptake in rural areas.

Association of Methamphetamine and Opioid Use With Nonfatal Overdose in Rural Communities

P. Todd Korthuis et al.

Summary: This study estimated the prevalence of methamphetamine use among people who use drugs in rural US communities and found that methamphetamine use was associated with nonfatal overdoses. The findings highlight the importance of reducing overdoses in rural communities by implementing harm reduction measures and substance use disorder treatment interventions that address both methamphetamine and opioids.

Evaluation of fentanyl test strip distribution in two Mid-Atlantic syringe services programs

Ju Nyeong Park et al.

Summary: Fentanyl test strip programs aim to reduce overdose risk among drug users by providing a method to test drugs for the presence of fentanyl. Findings from surveys conducted in Baltimore City and Delaware showed high utilization rates of FTS, adoption of risk reduction behaviors post-testing, and a strong interest in future FTS use among clients.

Ju Nyeong Park et al.

Summary: The study found that the presence of fentanyl in the illicit drug supply increased significantly in stimulants between 2011 and 2016, particularly with substantial increases occurring between 2015 and 2016; the presence of these products was concentrated primarily in the U.S. Northeast. There were significant variations in the proportions of fentanyl-cocaine and fentanyl-methamphetamine samples among different states.

Acceptability and Effectiveness of Hepatitis C Care at Syringe Service Programs for People Who Inject Drugs in New York City

Brandon Muncan et al.

Summary: The study found that people who inject drugs (PWID) generally perceive local syringe service programs (SSPs) as accessible and effective sites for hepatitis C (HCV) care. Key themes identified include the non-stigmatizing environment of SSPs, the role of SSPs in improving HCV knowledge, and the acceptability of SSPs for HCV care among PWID. This highlights the importance of understanding PWID attitudes towards SSPs in developing targeted strategies to reduce HCV transmission and achieve elimination goals.

Naloxone's role in the national opioid crisis- past struggles, current efforts, and future opportunities

Alex S. Bennett et al.

Summary: Naloxone has emerged as a critical overdose antidote in the past 25 years, but stigma and opposition still limit its full potential in reducing opioid-related mortality. Despite efforts to expand access to naloxone, it remains scarce in many US localities, with promising areas for expansion including telehealth distribution methods, lower barriers to pharmacy access, and collaboration with various entities to increase access. Efforts to ensure over-the-counter and low- or no-cost naloxone are ongoing, standing as efficacious and cost-efficient interventions in combating opioid-related overdose.

The rise of illicit fentanyls, stimulants and the fourth wave of the opioid overdose crisis

Daniel Ciccarone

Summary: The review provides an update on the rising trend of illicit fentanyls, overdose risks, and treatment options, particularly focusing on the increasing use of synthetic opioids like fentanyl and its analogs in the US. Methamphetamine and cocaine-driven stimulant-related overdoses are also on the rise nationwide, contributing to the 'fourth wave' of high mortality involving these substances. The use of illicit fentanyls remains a major contributor to overdose deaths, and the increase in stimulant-related deaths seems to be connected to the ongoing opioid epidemic.

The uptake of foil from needle and syringe provision services and its role in smoking or snorting heroin among people who inject drugs in Scotland

Karen Dunleavy et al.

Summary: This study utilized data from interviews with 1453 PWID conducted in Scotland in 2017-2018, finding that 36% of PWID had obtained foil from NSP services in the past six months. Those who received foil were more likely to smoke or snort heroin, and this was associated with lower likelihood of some risky injecting behaviors. Further research is needed to understand the motivations behind smoking drugs and obtaining foil from NSP services among PWID.
