PICaSSO Histologic Remission Index (PHRI) in ulcerative colitis: development of a novel simplified histological score for monitoring mucosal healing and predicting clinical outcomes and its applicability in an artificial intelligence system

Xianyong Gui et al.

Summary: Histological remission is emerging as a key treatment target in UC, and the development of a simple histological index PHRI showed high correlation with endoscopic activity and clinical outcomes. A deep learning AI system based on PHRI accurately predicted histological remission and differentiated active from quiescent UC, with promising sensitivity, specificity, and accuracy results.

Advanced technology for assessment of endoscopic and histological activity in ulcerative colitis: a systematic review and meta-analysis

Olga Maria Nardone et al.

Summary: Advanced endoscopic technologies play an important role in the diagnosis and treatment of ulcerative colitis. This study systematically reviewed and analyzed the correlations between different endoscope technologies and endoscopic and histological activity scores. The findings indicate that endoscopic activity scores are strongly correlated with histological activity regardless of the endoscopic technology used. Virtual chromoendoscopy appears to be more accurate in predicting histological remission compared to standard-definition white light.

Central Reading of Ulcerative Colitis Clinical Trial Videos Using Neural Networks

Klaus Gottlieb et al.

Summary: A deep learning algorithm was successfully trained to predict levels of UC severity from full-length endoscopy videos, with excellent agreement metrics compared to human central readers. Prospective data collection from a multinational clinical trial, use of videos instead of still images, and reporting of UCEIS and eMS all contributed to the success of the machine learning algorithm in predicting UC severity.

Pitfalls in training and validation of deep learning systems

Tom Eelbode et al.

Summary: The number of publications on deep learning applications in endoscopic journals has increased significantly in recent years, showing great promise. However, the interdisciplinary nature of these works has made it more difficult to evaluate their value and applicability. This review discusses common pitfalls and misconducts in training and validating deep learning systems, and proposes guidelines to ensure unbiased systems for routine clinical practice.
