Alveolar macrophage-derived progranulin mediated pro-inflammatory Il-6 expression via regulating Creb1 in silicosis model

Summary: This study demonstrated that PGRN levels were significantly increased in a silicosis mouse model. PGRN was mainly expressed in alveolar macrophages and promoted the transcription of pro-inflammatory cytokines, with IL-6 showing the most significant increase. Knockdown of PGRN blocked the elevation of IL-6 in silicosis-induced alveolar macrophages. Furthermore, PGRN was found to increase the phosphorylation of Creb1, which is a transcriptional regulator of IL-6. These findings suggest that PGRN may play a key role in silica-induced inflammation and provide new insights for the diagnosis and drug therapy of silicosis.

Progressive massive fibrosis: An overview of the recent literature

Summary: This review provides a comprehensive overview of the literature on progressive massive fibrosis (PMF), including its pathophysiology, epidemiology, impact on pulmonary function, imaging advances, and factors affecting progression to PMF in dust-exposed workers. The studies support the role of silica in causing PMF in coal miners and artificial stone workers. Computed tomography is more sensitive in detecting PMF than chest X-ray, while magnetic resonance imaging shows promise in differentiating PMF from lung cancer. The presence of small opacity pneumoconiosis increases the risk of progression to PMF. Effective new treatments for PMF are currently lacking.

This article summarizes the main causes of ATII cell dysfunction, describes the known mechanisms of physiological lung regeneration, and reviews the most relevant clinical trials, emphasizing the urgency of developing new therapies for IPF that promote ATII-driven alveolar regeneration.
