Summary: Chronic lymphocytic leukemia (CLL) is preceded by monoclonal B-cell lymphocytosis (MBL), a common condition in aged individuals. In this study, significant skewing in the B-cell receptor (BcR) immunoglobulin heavy chain (IGH) gene repertoire was found in the majority of CLL patients, even before the occurrence of lymphocytosis. Furthermore, some patients already had clonotypes associated with poor prognosis years before diagnosis.

The co-occurrence of SNVs/indels and CNAs affecting specific genes was associated with higher mortality from hematological malignancies, highlighting the importance of detecting both types of alterations in evaluating clonal hematopoiesis.
