Summary: Transient focal neurological episodes (TFNEs) are often overlooked presentations of cerebral amyloid angiopathy (CAA) and are associated with a high risk of lobar intracerebral hemorrhage (ICH) and death. Motor symptoms and antithrombotic use during follow-up are risk factors for ICH in patients with CAA-associated TFNEs.

Summary: The EMERGE and ENGAGE phase 3 trials provided data on ARIA associated with aducanumab treatment in patients with Alzheimer's disease. A high rate of ARIA was observed, with ARIA-edema being the most common adverse event. Some patients in the aducanumab group experienced associated symptoms, with headache being the most common.

The Interplay Between Brain Vascular Calcification and Microglia

Summary: Vascular calcifications refer to the ectopic deposition of calcium and phosphate in the blood vessels, often associated with pathological conditions. While peripheral organ vascular calcifications have been extensively studied, the understanding of vascular calcification in the central nervous system is limited. This review focuses on the occurrence of vessel calcifications in the brain in relation to aging and various brain diseases, using primary familial brain calcification as an example. The role of microglia in the brain's response to vessel calcification and their involvement in the clearance of calcifications are discussed.

Superficial Siderosis: A Clinical Review

Summary: Superficial siderosis of the central nervous system is caused by chronic low-grade bleeding into the subarachnoid space, resulting in hemosiderin deposition. Commonly presenting with gait ataxia and hearing impairment, dural disease, especially dural tears, is the most common etiology. Surgical correction of the bleeding source is a logical therapeutic strategy, with variable clinical outcomes reported.

Occipital Cortical Calcifications in Cerebral Amyloid Angiopathy

Summary: Cortical calcifications are common in Dutch-type hereditary CAA (D-CAA) but rare in sporadic CAA. These calcifications are associated with the development of intracerebral hemorrhage (ICH) and may serve as a marker for advanced CAA. Additionally, there seems to be a link between cortical calcifications on computed tomography and the striped occipital cortex on 7T-magnetic resonance imaging.

Summary: Transient focal neurologic episodes (TFNEs) are common in patients with cerebral amyloid angiopathy (CAA) and may be difficult to differentiate from TIAs. They predict a high rate of future lobar intracerebral hemorrhage, highlighting the importance of timely diagnosis and management. Cortical spreading depression triggered by acute or chronic superficial brain bleeding may be the underlying mechanism.

Spontaneous ARIA-like Events in Cerebral Amyloid Angiopathy-Related Inflammation A Multicenter Prospective Longitudinal Cohort Study

Summary: This study aimed to investigate the natural history and outcomes after treatment of spontaneous amyloid-related imaging abnormalities (ARIA)-like in cerebral amyloid angiopathy-related inflammation (CAA-ri). The results showed that most patients clinically and radiologically recovered within 3 to 12 months after the first-ever presentation of CAA-ri, but 38.3% experienced at least one recurrence within the following 24 months, with a higher likelihood of recurrence if IV high-dose corticosteroid pulse therapy was suddenly stopped.
