Diagnosis of single adhesive bands versus matted adhesions in small bowel obstructions: a radiological predictive score

Pierre Berge et al.

Summary: The study developed a radiological score to differentiate between SBA and MA as the etiology of SBO, which could be a significant tool in the decision for surgical management.

Impact of a procalcitonin-based algorithm on the quality of management of patients with uncomplicated adhesion-related small bowel obstruction assessed by a textbook outcome: a multicenter cluster-randomized open-label controlled trial

Charles Sabbagh et al.

Summary: Acute adhesion-related small bowel obstruction (ASBO) is a common digestive emergency, and the efficacy of conservative management is debatable. This study proposes the use of a procalcitonin (PCT)-based algorithm to aid the decision of whether or not to perform surgery, aiming to improve the quality of management for ASBO patients.

Surgical treatment of mechanical bowel obstruction: characteristics and outcomes of geriatric patients compared to a younger cohort

Christian J. J. Paul et al.

Summary: This study aimed to compare the characteristics and outcomes of geriatric and non-geriatric patients undergoing surgery for mechanical bowel obstruction. The results showed that geriatric patients had higher mortality and complication rates, as well as longer hospital stays. Geriatric patients with large bowel obstruction had a higher rate of bowel resection and stoma creation. Additionally, the time from admission to surgery did not affect the outcomes of (geriatric) patients.

Re-examining Never Letting the Sun Rise or Set on a Bowel Obstruction in the Era of Acute Care Surgery

Adrian Diaz et al.

Summary: Although the majority of SBO patients are treated non-operatively, older patients with more comorbidities who experience complications and longer hospital stays are more likely to undergo surgery. Hospitals with processes to prioritize urgent cases and surgeons covering EGS at multiple locations have lower odds of operation. Late operations are associated with increased complications and mortality.

Non-operative management for small bowel obstruction in a virgin abdomen: a systematic review

Tze W. W. Yang et al.

Summary: The study found that non-operative management is a safe and feasible option for small bowel obstruction in patients with a virgin abdomen, with adhesions being the most common cause of obstruction in this group. Further research is needed to support these findings with larger clinical studies and long-term follow-up data.

Non-operative management of small bowel obstruction in virgin abdomen: a systematic review

Nicole Hew et al.

Summary: In virgin abdomen small bowel obstruction patients, non-operative management was effective in resolving symptoms in majority of cases. Surgical intervention was mainly performed for cases with identifiable causes such as adhesions or malignancy. Further prospective studies are needed to better understand the feasibility and safety of non-operative management in this specific population.

Diagnosis and management of small bowel obstruction in virgin abdomen: a WSES position paper

Yousef Amara et al.

Summary: The etiology and treatment outcomes for patients with small bowel obstruction in the virgin abdomen (SBO-VA) are comparable to those in patients with SBO after previous abdominal surgery. A relatively high number of cases of SBO-VA are managed surgically, but non-operative management has also shown success. Therefore, patients with a virgin abdomen can potentially be treated according to existing guidelines for SBO and adhesive small bowel obstruction.
