Abdominal normothermic regional perfusion in controlled donation after circulatory determination of death liver transplantation: Outcomes and risk factors for graft loss

Amelia J. Hessheimer et al.

Summary: The study shows the benefits of using A-NRP over SRR in cDCD liver transplants in Spain, with reduced risks of overall biliary complications, ischemic type biliary lesions, graft loss, and patient death. Cold ischemia time and re-transplantation indication are significant independent predictors for graft loss in cDCD livers with A-NRP. Further improvements are needed for cases with prolonged cold ischemia and technically complex recipients, suggesting a potential role for complementary ex situ perfusion preservation techniques.

Predicting Early Allograft Function After Normothermic Machine Perfusion

Christopher J. E. Watson et al.

Summary: Analysis of perfusate variables from normothermic ex situ liver perfusion provides factors for predicting early allograft function, including 2-h alanine transaminase, 2-h lactate, amount of supplementary bicarbonate, and peak bile pH. Nonanastomotic biliary strictures affected 11% of transplants, highlighting the importance of further studies on cholangiocyte function markers.

National time trends in mortality and graft survival following liver transplantation from circulatory death or brainstem death donors

David Wallace et al.

Summary: This study compared the mortality and graft failure rates between recipients of livers donated after circulatory or brainstem death. The results showed a significant decrease in mortality rates for DCD recipients, which eventually became similar to that of DBD recipients. However, there was no improvement in graft failure rates for either DCD or DBD recipients. These findings suggest that the use of DCD livers may be a viable option in regions with high waiting list mortality.

Lysis of cold-storage-induced microvascular obstructions for ex vivo revitalization of marginal human kidneys

Summary: The study reveals a new mechanism of injury in marginal organs during cold storage, and suggests that normothermic machine perfusion combined with plasminogen and tissue plasminogen activator can improve organ conditions and has potential for translational benefits.

Thrombolytic Therapy During ex-vivo Normothermic Machine Perfusion of Human Livers Reduces Peribiliary Vascular Plexus Injury

Summary: This study demonstrated that administration of tPA into discarded DCD livers during NMP can effectively reduce PVP and mural stroma injury, providing a new method to decrease biliary complications. Further research is needed to evaluate the long-term impact of tPA administration on biliary complications.

Mitochondria as Therapeutic Targets in Transplantation

Kourosh Saeb-Parsy et al.

Summary: Advancements in surgical techniques, technology, and immune suppression have revolutionized organ transplantation, however, the metabolic changes during transplantation have been overlooked. Understanding the metabolic changes, especially those associated with mitochondria, could lead to improvements in transplanted grafts' function and longevity, ultimately increasing the pool of available organs for transplantation.

Summary: This study demonstrated that hypothermic oxygenated machine perfusion can reduce the risk of nonanastomotic biliary strictures following liver transplantation from donors after circulatory death, with lower rates of complications compared to conventional static cold storage.

Aggravation of fibrin deposition and microthrombus formation within the graft during kidney transplantation

Tamar A. J. van den Berg et al.

Summary: During kidney transplantation, the occurrence of microthrombi and fibrin deposition after reperfusion may lead to poorer initial graft function. It has been observed that deceased donor kidneys show a higher incidence of microthrombi and stronger fibrin intensity compared to living donor kidneys preimplantation. However, after reperfusion, both types of kidneys exhibit similar levels of microthrombi formation, indicating that microthrombi in living donor kidneys are not solely donor-derived.
