Comparison of endoscopic resection techniques for duodenal neuroendocrine tumors: systematic review

Summary: This review examined the effectiveness and complications of endoscopic techniques for resecting dNETs <= 20 mm in diameter. EMR-C or EMR-I were found to be preferred methods for resection, while polypectomy was not recommended due to a high rate of incomplete resections. ESD was associated with no recurrence but an increased risk of bleeding and perforation.

Summary: The study suggests that the main predictor of recurrence in low-grade DNETs <1.0 cm is the presence of positive tumor margins at the initial endoscopic resection. Ninety-two percent of recurrences were detected within 3 years of resection, indicating the need for more frequent, earlier surveillance. No recurrences occurred in the cohort after 4 years of surveillance.

Summary: Neuroendocrine tumours of the upper small intestine are rare, slow growing, and can be treated with endoscopic removal or surgery. Long-term survival is associated with tumor growth rate and complete tumor removal, with larger tumors potentially requiring surgery for lymph node metastases. Patients with these tumors generally have long overall survival, and both endoscopic and surgical resections have shown effective results with low recurrence rates.
