Antiphospholipid antibodies in women with recurrent embryo implantation failure: A systematic review and meta-analysis

Marina Jarne-Borras et al.

Summary: Through a systematic review and meta-analysis, we found that different types of antiphospholipid antibodies are present in women with recurrent implantation failure. Among them, IgG anticardiolipin autoantibodies are associated with recurrent implantation failure. Additionally, anti-beta 2 glycoprotein I-IgA, antiphosphatidylglycerol-IgG, and antiphosphatidylglycerol-IgM are also associated with recurrent implantation failure.

Appraising screening, making risk in/visible. The medical debate over Non-Rare Thrombophilia (NRT) testing before prescribing the pill

Mauro Turrini et al.

Summary: The controversy surrounding non-rare thrombophilia (NRT) revolves around the debate on whether genetic testing should be prescribed and how risks should be evaluated. Advocates of selective screening believe that genetic tests provide a better estimation of the risk, while proponents of universal or 'extended' screening see these tests as a moral/legal obligation. Risk visibility is crucial in analyzing the complex setting that involves clinical, political, social, and cultural considerations.

Approach to the Evaluation and Treatment of Venous Thromboembolism in Pregnancy

Benjamin Brenner et al.

Summary: Thrombosis in pregnancy is a major cause of maternal and fetal morbidity and mortality, with complex risk stratification for VTE during pregnancy. The hypercoagulability in pregnant women can reduce bleeding during childbirth but also increase the risk of thrombosis. Treatment of VTE in pregnancy primarily relies on low-molecular-weight heparins, with dosing adjustments based on individual factors.

Ovarian Hyperstimulation: Diagnosis, Prevention, and Management

Paolo Emanuele Levi-Setti et al.

Summary: Ovarian hyperstimulation syndrome (OHSS) is a severe complication of controlled ovarian stimulation (COS) characterized by the transudation of protein-rich fluid, which can be easily avoided by identifying high-risk patients, choosing the correct stimulation protocol, and using the freeze-all strategy.

Prevention and management of venous thromboembolism in pregnancy: cutting through the practice variation

Leslie Skeith

Summary: There is clinical practice variation in the prevention and management of VTE during pregnancy, especially regarding the use of postpartum LMWH. Controversial topics include LMWH dosing, anti-Xa level monitoring, and peripartum anticoagulation management.

Effect of blood loss during caesarean section on coagulation parameters

A. Wasserloos et al.

Summary: In pregnant women undergoing caesarean section, moderate blood loss does not significantly reduce thrombin generation but exacerbates pregnancy-induced resistance to activated protein C and combined deficiency of tissue factor pathway inhibitor and protein S, which may account for the increased risk of thrombosis in early postpartum.

The Maribor consensus: report of an expert meeting on the development of performance indicators for clinical practice in ART

Veljko Vlaisavljevic et al.

Summary: The study recommends six performance indicators for monitoring clinical work in assisted reproductive technology, including cycle cancellation rate, ovarian hyperstimulation syndrome rate, proportion of mature oocytes at ICSI, complication rate after oocyte pick-up, clinical pregnancy rate, and multiple pregnancy rate. These indicators aim to enhance the quality of ART practice and are essential for managing staff performance.

Thrombophilia, Inflammation, and Recurrent Pregnancy Loss: A Case-Based Review

Elvira Grandone et al.

Summary: Recurrent pregnancy loss (RPL) is defined as the loss of two or more pregnancies, often due to factors such as aneuploidy, anatomic or physiological abnormalities, and potentially inherited or acquired thrombophilias. Recent research has also suggested a role for systemic inflammation in infertility. Antithrombotic therapy and antiplatelet agents have been proposed as possible tools for preventing RPL, but the multifactorial nature of the condition leads to different diagnostic and therapeutic recommendations among clinicians of various specialties.
