Ambulatory Blood Pressure Monitoring in Children and Adolescents: 2022 Update: A Scientific Statement From the American Heart Association

Joseph T. Flynn et al.

Summary: The use of ambulatory blood pressure monitoring in children and adolescents has increased significantly, as has the evidence base for its use. This scientific statement provides a concise review of the new evidence, guidance on optimal application of ambulatory blood pressure monitoring in the clinical setting, and an updated classification scheme for interpretation. Areas of uncertainty requiring further research are also highlighted.

Discordances between pediatric and adult thresholds in the diagnosis of hypertension in adolescents with CKD

Elizabeth Black et al.

Summary: Adolescents with chronic kidney disease have a high prevalence of hypertension. Different definitions of hypertension in pediatric and adult populations show some variations in predicting cardiovascular and renal outcomes, with slightly better risk discrimination for left ventricular hypertrophy using pediatric definitions.

Ambulatory hypertension diagnosed by 24-h mean ambulatory versus day and night ambulatory blood pressure thresholds in children: a cross-sectional study

Ajay P. Sharma et al.

Summary: This study evaluated the agreement between different ambulatory blood pressure (ABP) thresholds in diagnosing ambulatory hypertension, white coat hypertension (WCH), and masked hypertension (MH) in children. The results showed that the 24-h ABP threshold classified fewer patients as ambulatory hypertension compared to day-night ABP thresholds. The American Heart Association (AHA) threshold exhibited a stronger agreement with the 24-h ABP threshold in diagnosing ambulatory hypertension.

Diagnosis of Pediatric Hypertension: European Society of Hypertension-Recommended 24-Hour vs. 24-Hour-Day-Night Ambulatory Blood Pressure Thresholds

Ajay P. Sharma et al.

Summary: In untreated children, the 24-hour ESH and combined ESH thresholds have similar agreement for diagnosing hypertension, but the combined ESH threshold has significantly higher AUC in office hypertension patients.

Challenges of diagnosing pediatric hypertension using ambulatory blood pressure monitoring

Jason Thomas et al.

Summary: The study demonstrates that diagnosing pediatric hypertension using mean arterial pressure (MAP) and systolic/diastolic blood pressure shows good agreement, and using adult criteria for diagnosing hypertension in male children >= 165 cm may improve accuracy.

Reclassification of adolescent hypertension by ambulatory blood pressure monitoring using adult norms and association with left ventricular hypertrophy

Jessica Fallon Campbell et al.

Summary: The study evaluated the reclassification of hypertension in patients aged 13 years and older when applying adult ambulatory blood pressure monitor norms, and assessed the association of hypertension with left ventricular hypertrophy. The results showed significant differences in hypertension reclassification with adult norms, significant correlation between left ventricular mass index and systolic blood pressure, and significant association of hypertension with left ventricular hypertrophy.

Diagnosis of hypertension: Ambulatory pediatric American Heart Association/European Society of Hypertension versus blood pressure load thresholds

Ajay P. Sharma et al.

Summary: This study evaluated the agreement between traditional ABP-load thresholds and AHA/ESH pediatric ABP thresholds for diagnosing hypertension, showing stronger agreement between AHA/ESH thresholds and ABP load 40% and 50%, but weaker agreement with ABP load 25% and 30%. Further outcome-based studies are needed to determine optimal clinical use of these ABP thresholds.

Comparison of Pediatric and Adult Ambulatory Blood Pressure Monitoring Criteria for the Diagnosis of Hypertension and Detection of Left Ventricular Hypertrophy in Adolescents

Kumail Merchant et al.

Summary: The study found that adult ABPM criteria outperformed pediatric criteria in diagnosing hypertension and detecting LVH in adolescents, suggesting a consideration to align ABPM criteria for diagnosing hypertension in adolescents with adult guidelines is warranted.

Pediatric and Adult Ambulatory Blood Pressure Thresholds and Blood Pressure Load as Predictors of Left Ventricular Hypertrophy in Adolescents

Gilad Hamdani et al.

Summary: Adult and pediatric standards differ in predicting LVH, with adult cut points being more sensitive. Pediatric standards are usually based on sex and height, while adult cut points are more effective.
