Association Between Orthostatic Hypotension and Dementia in Patients With Parkinson Disease and Multiple System Atrophy

Summary: This study aims to evaluate the association between orthostatic hypotension (OH) and dementia risk in patients with Parkinson disease (PD), as well as cognitive impairment risk in patients with multiple system atrophy (MSA). The study found that early OH increases the risk of cognitive impairment in patients with PD and MSA, but not its symptom severity. Further research is needed to determine if treating OH can modify the risk of dementia in these conditions.

Cognitive profile in idiopathic autonomic failure: relation with white matter hyperintensities and neurofilament levels

Summary: This study aims to disclose the nature of cognitive deficits in patients with idiopathic autonomic failure (IAF) and explore their relation to cardiovascular autonomic failure (AF) and phenoconversion. The results showed that approximately 43% of IAF patients displayed cognitive deficits, mainly affecting executive functioning. The presence of cognitive deficits was not associated with phenoconversion or autonomic function parameters, but correlated with cerebral white matter hyperintensities (WMH) load and neurofilament light chain (NfL) levels.

Interventions targeting blood pressure reduction, lifestyle changes, and promoting microvascular health have the potential to prevent or delay vascular cognitive impairment and Alzheimer's disease in hypertensive patients.
