High Prevalence of Plasmodium falciparum K13 Mutations in Rwanda Is Associated With Slow Parasite Clearance After Treatment With Artemether-Lumefantrine

Judith Straimer et al.

Summary: Mutations in the Plasmodium falciparum K13 gene have caused treatment failures in malaria patients receiving artemisinin combination therapies in Southeast Asia. Until recently, relevant K13 mutations had been mostly absent from Africa. A phase 2 clinical study in Mali, Gabon, Ghana, Uganda, and Rwanda reported a 22% allele frequency for R561H in Rwanda and associated delayed parasite clearance.

Association of Plasmodium falciparum kelch13 R561H genotypes with delayed parasite clearance in Rwanda: an open-label, single-arm, multicentre, therapeutic efficacy study

Summary: This study confirms the emergence of artemisinin partial resistance in Rwanda. Although artemether-lumefantrine remains efficacious, vigilance for decreasing efficacy and further characterization of artemisinin partial resistance should be considered. Evaluation of additional antimalarials in Rwanda is also recommended.

Efficacy and safety of artemether-lumefantrine for treatment of uncomplicated Plasmodium falciparum malaria in Ethiopia: a systematic review and meta-analysis

Abdulhakim Abamecha et al.

Summary: This study aimed to synthesize the evidence on the efficacy of artemether-lumefantrine for uncomplicated falciparum malaria in Ethiopia. The results suggest that this therapy is effective and safe, but strong conclusions cannot be drawn due to high risk of bias in the included studies. Further high-quality RCTs are needed to provide stronger evidence.

Evidence of Artemisinin-Resistant Malaria in Africa

Betty Balikagala et al.

Summary: Research conducted in Northern Uganda revealed the emergence of artemisinin-resistant Plasmodium falciparum strains, with predominant mutations found in the kelch13 gene. Thirteen patients were infected with P. falciparum parasites carrying mutations in the A675V or C469Y allele, which were associated with prolonged parasite clearance half-lives.
