Dynamic impact of liver allocation policy change on donor utilization

Ethan Chan et al.

Summary: The liver allocation policy was changed in February 2020 to reduce variability in median MELD scores at transplant. The impact of this policy change on donor utilization is crucial. Centers implemented various strategies to broaden organ acceptance criteria and increase transplant volume.

A multicentre outcome analysis to define global benchmarks for donation after circulatory death liver transplantation

Andrea Schlegel et al.

Summary: The study established the best possible outcomes and reference values for liver transplantation using grafts donated after circulatory death (DCD) through benchmarking. The benchmark cases showed excellent 1-year retransplant and mortality rates, but had high rates of major complications. The use of organ perfusion technology reduced complications and improved outcomes in high-risk patients.

Survival After Liver Transplantation: An International Comparison Between the United States and the United Kingdom in the Years 2008-2016

Tommy Ivanics et al.

Summary: Compared to the United States, the United Kingdom has seen improvements in short-term and long-term mortality rates following liver transplantation. This study aimed to compare the mortality rates of liver transplant recipients in both countries and found that the United Kingdom had lower mortality rates after 90 days. This international comparison study helps identify differences in healthcare delivery and provides benchmarking for improved global outcomes in liver transplantation.

Impact of the acuity circle model for liver allocation on multivisceral transplant candidates

Tommy Ivanics et al.

Summary: The update in liver allocation from DSA to AC had varying impacts on waitlist mortality and transplant probability for patients listed for combined liver-intestine transplantation, with lower transplant probability observed in adults but similar outcomes in pediatric patients. Additionally, there was a decrease in the proportion of patients who underwent transplant with exception points post-AC. Efforts should be made to ensure equitable organ allocation in this vulnerable patient population.

The Effect of Acuity Circles on Deceased Donor Transplant and Offer Rates Across Model for End-Stage Liver Disease Scores and Exception Statuses

Andrew Wey et al.

Summary: The new liver allocation system, Acuity circles (AC), has a positive impact on the transplant rates of candidates with higher PELD/MELD scores, while exceptions can affect offer rates. The quality of liver donors may see minor changes with the use of donation after circulatory death (DCD) donors.

Donation after circulatory death transplant outcomes using livers recovered by local surgeons

Caroline C. Jadlowiec et al.

Summary: The outcomes of donation after circulatory death (DCD) liver transplantation (LT) were analyzed based on different graft recovery techniques. The study found that excellent outcomes can be achieved using non-TPT for the recovery of DCD livers, which suggests the potential to expand the use of DCD livers in the United States.

Disparities in the Use of Older Donation After Circulatory Death Liver Allografts in the United States Versus the United Kingdom

Emmanouil Giorgakis et al.

Summary: There are differences between the United States and the United Kingdom in terms of characteristics and posttransplant survival of patients who received donation after circulatory death (DCD) liver allografts from donors aged >60 years. The utilization of older DCD liver transplants is low in the United States, resulting in worse survival for donors aged >60. However, survival rates for donors aged <= 60 are similar in both countries, supporting an extension to the current US DCD age cutoff.

National time trends in mortality and graft survival following liver transplantation from circulatory death or brainstem death donors

David Wallace et al.

Summary: This study compared the mortality and graft failure rates between recipients of livers donated after circulatory or brainstem death. The results showed a significant decrease in mortality rates for DCD recipients, which eventually became similar to that of DBD recipients. However, there was no improvement in graft failure rates for either DCD or DBD recipients. These findings suggest that the use of DCD livers may be a viable option in regions with high waiting list mortality.

Summary: The study found that receiving a re-LT in the post-DAA era was associated with improved patient and graft survival among HCV patients, while there was no significant change observed in survival rates for non-HCV patients.

OPTN/SRTR 2019 Annual Data Report: Liver

A. Kwong et al.

Summary: The year 2019 saw significant changes in liver transplant policies, including adjustments to exception points and priority systems, the implementation of a National Liver Review Board, and a short-lived national acuity circle liver distribution policy. Additionally, there was a continued increase in living-donor liver transplants, with lower transplant rates but higher pretransplant mortality rates for women.

Centre volume impact on graft survival and waiting list time in donation after circulatory death liver transplantation in the USA

Emmanouil Giorgakis et al.

Summary: From 2001 to 2015, the number of DCD liver transplantations remained surprisingly low, but post-transplant outcomes improved overall. High-volume centers promoting DCD LT significantly reduced waiting list time, thereby limiting waiting list mortality and drop-outs.
