Faecal microbiota replacement to eradicate antimicrobial resistant bacteria in the intestinal tract - a systematic review

Manu P. Bilsen et al.

Summary: Recent studies have shown some effectiveness of fecal microbiota transplantation (FMT) for decolonization of multidrug-resistant organisms (MDRO), but questions remain regarding its true efficacy, optimal route of administration, role of antibiotics pre and post-FMT, and efficacy in different patient populations. Further research is needed to explore the observed decrease in MDRO infections post-FMT.

Rectal colonization by resistant bacteria increases the risk of infection by the colonizing strain in critically ill patients with cirrhosis

Summary: Rectal colonization by MDROs is extremely frequent in critically ill patients with cirrhosis, and colonization increases the risk of infection by the colonizing resistant strain.

Global burden of bacterial antimicrobial resistance in 2019: a systematic analysis

Summary: Antimicrobial resistance (AMR) poses a significant threat to global human health, and this study provides the most comprehensive estimates of AMR burden to date. By estimating deaths and disability-adjusted life-years (DALYs) associated with bacterial AMR in 204 countries and territories in 2019, the study highlights the impact of resistance and the leading pathogen-drug combinations contributing to it.

Summary: The study found that NDM-Kp is associated with a higher risk of BSI compared to KPC-Kp, and this association seems to be strongly related to the high-risk clone ST147.

Compositional Flux Within the Intestinal Microbiota and Risk for Bloodstream Infection With Gram-negative Bacteria

Igor Stoma et al.

Summary: This study investigated the connection between gram-negative intestinal colonization and the risk of gram-negative BSIs in allo-HCT recipients. Results showed that domination by gram-negative bacteria in the intestine was associated with subsequent BSI. Fluoroquinolone prophylaxis seemed to reduce these infections by influencing gut colonization. Initial colonization by certain genus members was associated with protection against gram-negative BSI.

Risk Factors and Impact of Perioperative Prophylaxis on the Risk of Extended-spectrum β-Lactamase-producing Enterobacteriaceae-related Infection Among Carriers Following Liver Transplantation

Elsa Logre et al.

Summary: ESBL-E carriage is common among liver transplant recipients, but infections only occur in a small percentage. Predictors for post-LT ESBL-E infection include Klebsiella pneumoniae carriage, model for end-stage liver disease >= 25, preoperative spontaneous bacterial peritonitis prophylaxis, and antimicrobial exposure during the previous month. Perioperative antimicrobial prophylaxis targeting colonizing ESBL-E may reduce the risk of post-LT ESBL-E infections.

Risk-of-bias VISualization (robvis): An R package and Shiny web app for visualizing risk-of-bias assessments

Luke A. McGuinness et al.

Summary: Despite the availability of numerous software options for evidence synthesis, there is currently no universal tool for generating figures to display and explore risk-of-bias assessments. However, tools such as the R programming environment and Shiny have made it easy to create new tools for producing evidence syntheses. The new tool robvis facilitates the rapid production of publication-quality risk-of-bias assessment figures and is available as an R package and web app.

Methodological quality of studies evaluating the burden of drug-resistant infections in humans due to the WHO Global Antimicrobial Resistance Surveillance System target bacteria

Maria Diletta Pezzani et al.

Summary: This study assessed the clinical impact of infections caused by priority antibiotic resistant pathogens monitored by the WHO. The results show the need for improved data harmonization, representativeness, and quality in surveillance to accurately estimate the burden of AMR. Policy makers require accurate burden estimates to prioritize resource allocation and intervention strategies for combating the AMR crisis.
