Hematopoietic Stem Cell Transplant-M embranous Nephropathy Is Associated with Protocadherin FAT1

Summary: In this study, a novel antigen associated with membranous nephropathy (MN) in patients receiving hematopoietic stem cell transplant (HSCT) was identified using laser microdissection and tandem mass spectrometry (MS/MS). The antigen, called FAT1, was detected in the glomeruli of patients with PLA2R-negative MN. Further studies confirmed the presence of FAT1-associated MN in a majority of HSCT-related MN cases. This study provides valuable insights into the pathogenesis of MN in HSCT patients and may contribute to the development of targeted therapies.

Nell1 as Target Antigen for Mercury Related Membranous Nephropathy: A Case Report

Summary: Membranous nephropathy accounts for 30% of adult nephrotic syndrome cases, with primary membranous nephropathy being the most common. Mercury exposure is known to cause secondary membranous nephropathy, although the exact pathophysiology of mercury-related membranous nephropathy is not yet understood. We reported a case of NELL-1 positive mercury-related membranous nephropathy that showed improvement after cessation of siddha medication and antiproteinuric treatment, along with concomitant euthyroid lymphocytic thyroiditis suggesting a systemic autoimmune phenomenon due to mercury exposure.

NELL1 is a target antigen in malignancy-associated membranous nephropathy

Summary: Patients with membranous nephropathy associated with malignancy were found to have unique NELL1 immune complexes in kidney biopsy tissue, with characteristic histopathology of incomplete capillary loop staining and IgG1 predominance. These NELL1-positive cases also showed a higher association with concurrent malignancy compared to other known types of membranous nephropathy.

Neural cell adhesion molecule 1 is a novel autoantigen in membranous lupus nephritis

Summary: Research has identified neural cell adhesion molecule 1 (NCAM1) as a target antigen in some cases of membranous lupus nephritis and rare cases of primary membranous nephropathy, colocalizing with IgG in glomerular immune deposits. Circulating antibodies to NCAM1 have been detected in serum of patients with NCAM1-associated membranous nephropathy, suggesting diagnostic significance. Future studies are needed to determine whether anti-NCAM1 antibody levels correlate with disease activity or response to therapy.

In Patients with Membranous Lupus Nephritis, Exostosin-Positivity and Exostosin-Negativity Represent Two Different Phenotypes

Lipoic acid supplementation associated with neural epidermal growth factor-like 1 (NELL1)-associated membranous nephropathy

Summary: Lipoic acid, a popular antioxidant and insulin-mimetic supplement, was found to cause high-grade proteinuria in multiple sclerosis patients in a clinical trial. This led to the discovery of NELL1-associated membranous nephropathy cases related to lipoic acid supplementation, which improved with discontinuation of the supplement and supportive therapy.

Serine Protease HTRA1 as a Novel Target Antigen in Primary Membranous Nephropathy

Summary: The study identified serine protease HTRA1 as a novel podocyte antigen in primary membranous nephropathy (MN) patients. Anti-HTRA1 antibodies in patients with HTRA1-associated MN were predominantly IgG4 and seemed to correlate with clinical disease activity. Screening of 118 quadruple-negative patients revealed a 4.2% prevalence of HTRA1-associated MN.

Summary: Technological advances have led to the discovery of 6 subtypes of membranous nephropathy based on target antigens, including M-type phospholipase A2 receptor (PLA(2)R) and thrombospondin type 1 domain-containing 7A (THSD7A). This study presents two cases of primary membranous nephropathy with enhanced glomerular staining of EXT1/EXT2, both of which achieved clinical remission with immunosuppressive therapy, despite the absence of autoimmune diseases and antinuclear antibodies in their sera.
