A clinical perspective on endovascular stroke treatment biomechanics

Johanna M. Ospel et al.

Summary: Acute ischemic stroke is caused by a thrombus blocking a cerebral artery, leading to oxygen supply interruption. Factors influencing endovascular treatment success can be grouped into vessel, clot, technique, and operator-related categories. While some factors are non-modifiable, research is ongoing to optimize modifiable factors for treatment success.

Quantitative 3D analysis of tissue damage in a rat model of microembolization

Theodosia Georgakopoulou et al.

Summary: In this study, a rat model of microembolization was used to investigate the spatial correlation between microspheres and induced hypoxia or infarction. Results showed that multiple microspheres can lead to local areas of hypoxia or infarction when lodged in large numbers, despite individual microemboli not always being harmful.

Modeling acute ischemic stroke recanalization through cyclic aspiration

Oyekola Oyekole et al.

Summary: In this study, we simulated the deformation of a thromboembolus lodged in a cerebral artery under the application of aspiration pressure during a mechanical thrombectomy procedure. The results showed that the application of aspiration pressure can cause the thrombus to impinge against the inner arterial wall, potentially leading to interfacial failure. These findings provide valuable information for identifying the crucial factors involved in clot removal by aspiration alone.

Blood clot fracture properties are dependent on red blood cell and fibrin content

Behrooz Fereidoonnezhad et al.

Summary: This study provides experimental characterization of the composition-dependent fracture properties of blood clots, showing that fracture toughness significantly increases with higher fibrin content. Mechanical behavior of clot analogues differs in compression and tension. Cohesive zone modeling reveals a toughening mechanism with aligned fibrin fibers. The results have implications for developing thrombectomy devices and clinical strategies.

The first virtual patient-specific thrombectomy procedure

Giulia Luraghi et al.

Summary: The recent improvement in the treatment of acute ischemic stroke is attributed to the introduction of endovascular mechanical thrombectomy, a minimally invasive procedure that can remove a clot using aspiration devices and/or stent-retrievers. Through computational studies, an accurate representation of the real thrombectomy procedure was achieved, highlighting the need for optimization planning and future improvements in stent designs.

Applicability analysis to evaluate credibility of an in silico thrombectomy procedure

Giulia Luraghi et al.

Summary: The study aims to apply a credibility process for the validation phases of intra-arterial thrombectomy procedure, in order to make it usable for in silico trials. Validation evidence is proposed to build credibility to the numerical model, and the applicability of the model is justified and assessed using a step-by-step method based on the ASME V&V40 protocol.

Impact of the Internal Carotid Artery Morphology on in silico Stent-Retriever Thrombectomy Outcome

Sara Bridio et al.

Summary: The study proposes a methodology to identify relationships between cerebral vasculature morphology and thrombectomy simulation outcomes for acute ischemic stroke. It finds that a combination of local geometric parameters is more effective in discriminating successful and unsuccessful simulations than global parameters. This promising methodology may eventually guide decision making in the treatment of acute ischemic stroke.

Applicability assessment of a stent-retriever thrombectomy finite-element model

Giulia Luraghi et al.

Summary: Mechanical thrombectomy is an effective treatment for acute ischaemic stroke, yet further investigation is required. A numerical finite-element-based model was developed in this study and validated through simulations using different vessel geometries. The results confirmed the efficacy of the model in replicating experimental setups.
