Endovascular therapy with or without intravenous thrombolysis in acute stroke with tandem occlusion

Summary: This study compared the treatment effects of EVT alone and EVT plus IVT in patients with acute ischemic stroke due to anterior tandem occlusions. The results showed higher odds of favorable and excellent outcomes, as well as successful reperfusion in the IVT+ group, without a difference in the risk of significant hemorrhagic complications. Secondary analysis also indicated better outcomes with bridging therapy in patients treated with acute cervical internal carotid artery stenting.

Endovascular Treatment for Posterior Circulation Stroke: Ways to Maximize Therapeutic Efficacy

Summary: The efficacy of endovascular treatment (EVT) in posterior circulation stroke (PC-EVT) is still uncertain. While some studies have shown no improvement in functional outcomes with EVT for PC-EVT, there are promising results from recent trials. Factors such as baseline clinical severity, infarct volume, collaterals, and time metrics need to be further studied to identify prognostic factors beyond recanalization. Additionally, individualized patient selection for PC-EVT and technical considerations for improving treatment efficacy are discussed.

Thrombectomy in acute ischemic stroke in the extended time window: Real-life experience in a high-volume center
