Biomechanical mechanisms of atypical femoral fracture

Summary: Antiresorptives, such as bisphosphonates and denosumab, effectively prevent osteoporotic fractures but long-term use may lead to rare atypical femoral fractures (AFF), causing patient anxiety and decreased prescription of osteoporosis treatment drugs. Current understanding of the pathogenesis of AFF is limited, with gaps in knowledge on the mechanical mechanisms underlying this condition.

Incidence of Hip and Subtrochanteric/Femoral Shaft Fractures in Postmenopausal Women With Osteoporosis in the Phase 3 Long-Term Odanacatib Fracture Trial

Summary: This study prospectively assessed femur fractures in women at increased fracture risk during treatment with the cathepsin K inhibitor, odanacatib (ODN), and found that the rates of hip and ST/FS fractures were lower in the ODN group compared to the placebo group, but low-energy ST/FS fractures were more frequent in ODN-treated women. The results provide insight into possible pathogeneses of atypical femur fractures, suggesting a need to reconsider the current diagnostic criteria.

Dose-dependent effects of pharmaceutical treatments on bone matrix properties in ovariectomized rats

Summary: Ovariectomized rats treated with high doses of parathyroid hormone and alendronate showed significantly increased cortical bone fracture toughness, primarily due to increased non-collagenous proteins with no change in AGEs. Additionally, low-dose parathyroid hormone treatment increased matrix crystallinity and decreased AGE levels. Further investigation is needed to study different doses and longer treatment periods.
