Comparison of Four International Guidelines on the Utility of Cranial Imaging Before Lumbar Puncture in Adults with Bacterial Meningitis

Summary: This study compared the ability of four international guidelines to identify intracranial abnormalities in adult patients with meningitis. The results showed that only the Infectious Diseases of America (IDSA) guidelines did not miss any major intracranial abnormalities or findings that required neurosurgical intervention, but all guidelines were able to identify cerebral herniation.

Pneumococcal Meningitis in Adults: A Prospective Nationwide Cohort Study Over a 20-year Period

Summary: The epidemiology and treatment of pneumococcal meningitis in the Netherlands has changed with the implementation of conjugate vaccines and adjunctive dexamethasone therapy, leading to a decrease in incidence of certain serotypes and an increase in the use of dexamethasone therapy. Although dexamethasone therapy was associated with a favorable outcome, pneumococcal meningitis still remains associated with a high rate of mortality and morbidity, highlighting the need for continuous efforts in identifying new adjunctive treatments.

Summary: Group B streptococcus (GBS) is a significant cause of life-threatening disease in newborns. Pregnant women can transmit GBS to their developing fetus and neonates, leading to severe infections and death. In some countries, intrapartum antibiotic prophylaxis (IAP) is the standard preventive measure, but a substantial disease burden remains. Vaccination during pregnancy could potentially address this unmet medical need for the prevention of invasive GBS disease in neonates.

Comparison of a commercial real-time PCR panel to routine laboratory methods for the diagnosis of meningitis-encephalitis

Summary: This study compared the accuracy and turnaround time of the BioFire FilmArray Meningitis/Encephalitis multiplex PCR panel to traditional testing algorithms for the diagnosis of meningitis-encephalitis. The ME panel showed high concordance with traditional testing, streamlined the laboratory workflow, and significantly reduced turnaround time. However, the failure of the ME panel to detect Cryptococcus spp. suggests the need for additional testing methods in cases where cryptococcal meningitis is suspected. Despite molecular assays targeting common causes of CNS infection, the diagnostic yield remains suboptimal.
