Impact of Contrast Enhancement and Virtual Monoenergetic Image Energy Levels on Emphysema Quantification Experience With Photon-Counting Detector Computed Tomography

Summary: This study evaluated the impact of contrast enhancement and different virtual monoenergetic image energies on automatized emphysema quantification with PCD-CT. The results showed that different energy images had a significant effect on noise and contrast-to-noise ratio. In lung parenchyma and pulmonary vessel evaluation, the highest subjective assessment scores were obtained at 70 keV. The best trade-off for emphysema assessment while maintaining reasonable contrast for pulmonary vessel evaluation was found between 60-70 keV. Overall, contrast-enhanced imaging led to underestimation of emphysema, and virtual noncontrast images served as a reference standard for emphysema quantification.

SPCCT showed higher detectability of CAC for both small and large phantoms, with detection increasing significantly at reduced slice thickness for both CT systems. Volume scores from SPCCT were found to be more comparable with the physical volume of the CAC compared to CT.

Summary: The study aimed to compare the objective and subjective image quality of high-pitch PCD-CT and conventional EID-CT of the aorta at the same radiation dose. The results showed that high-pitch PCD-CT with VMI at 40 and 45 keV had significantly higher CNR compared to EID-CT with ATVS at matched radiation dose. The CNR gain of PCD-CT was more pronounced in overweight patients. A trade-off between objective and subjective image quality was found with VMI at 45 to 50 keV.
