Clinical Features and Outcomes of Invasive Fusariosis: A Case Series in a Single Center with Literature Review

A study conducted at Samsung Medical Center in Korea reported 57 cases of culture-proven fusariosis, with a mortality rate of 37%. Clinical features of invasive fungal disease in children with no underlying disease

Summary: Limited research has been done on children with no underlying disease who suffer from invasive fungal disease (IFD). In this retrospective study conducted in Changsha, China, 9 children with IFD and no underlying disease were identified. Elevated eosinophil counts and procalcitonin levels were observed in 6 cases. Fungal culture was positive in all patients, with Cryptococcus neoformans being the most common pathogen. After antifungal treatment, the body temperature of patients with cryptococcal disease returned to normal in an average of 8.33 days. This study suggests that eosinophils can be considered as indicators of cryptococcal infection and that the prognosis of IFD in children with no underlying disease is satisfactory.

Antifungal drug resistance: an update

Summary: The limited number of antifungal classes and increasing resistance highlight the need for more focus on susceptibility testing and antifungal stewardship, which have proven to improve survival and overall clinical outcomes. Positive diagnosis of fungal infections through laboratory markers can allow for discontinuation of non-specific antibacterial therapy, contributing to antimicrobial resistance control. Non-culture based diagnostics for fungal diseases offer great sensitivity and speed, but are less effective in identifying antifungal resistance.

Machine learning-based in-hospital mortality prediction of HIV/AIDS patients with Talaromyces marneffei infection in Guangxi, China

Summary: This study focused on the use of machine learning models to predict the treatment outcomes of HIV/AIDS patients with T. marneffei infection in a hospital in Nanning, Guangxi. The XGBoost model showed the best predictive performance with an AUC of 0.97 in the training dataset and 0.90 in the testing dataset. High CD4/CD8 ratios, AST levels, and other factors were associated with in-hospital mortality in these patients.

Candida auris Bloodstream Infection Induces Upregulation of the PD-1/PD-L1 Immune Checkpoint Pathway in an Immunocompetent Mouse Model

Summary: Fungal pathogens are a significant global disease burden. However, their status as eukaryotes complicates their involvement in fungal-mediated infections and treatment. Antifungal drugs can be toxic to host cells, and there is increasing evidence of adaptive resistance in animals and humans. Current diagnostic and treatment methods have limitations, leading to high mortality rates and prolonged illness in immunocompromised patients. Therefore, real-time monitoring and diagnostic methods for fungal pathogens are needed, as well as increased awareness of the contribution of fungal pathogens to disease. More information is also needed on the selection and dosage of antifungal drugs, as well as factors governing resistance, to find more appropriate and effective solutions. Additionally, there is growing interest in developing alternative and complementary green biocides and disinfection innovations.

Intrinsic resistance to antifungals makes successful recovery difficult. Rapid diagnosis, surgical debridement, and timely antifungal treatment are key for successful management. Further research is needed to understand the environmental niche, pathophysiology, and species-specific therapeutic targets of this fungus.

The importance of antimicrobial resistance in medical mycology

The rise of antifungal drug resistant strains has worsened the impact of fungal infections on human health. This review aims to illuminate the problem of antifungal resistance and suggest possible measures to mitigate this growing threat.
