Single-cell methods in myeloproliferative neoplasms: old questions, new technologies

Jennifer Mary O'Sullivan et al.

Summary: Myeloproliferative neoplasms (MPN) are a group of hematopoietic malignancies driven by aberrant JAK/STAT signaling. Cellular heterogeneity at the single-cell level provides new insights into MPN biology, which can help predict clinical course and improve treatment strategies for these cancers, leading to better outcomes for patients.

Genetic basis and molecular profiling in myeloproliferative neoplasms

Damien Luque Paz et al.

Summary: BCR-ABL1-negative MPNs are clonal hematopoietic diseases characterized by excessive production of mature blood cells. The three subtypes include PV, ET, and PMF, diagnosed based on WHO and ICC criteria. Acquired gain-of-function mutations in JAK2, CALR, and MPL are causative events that activate the JAK/STAT signaling pathway. Limiting factors in MPN development include clone expansion, inherited predisposition, additional mutations, and inflammation. Understanding the mutational landscape in individual patients is important for predicting outcomes and choosing optimal therapy.

Life histories of myeloproliferative neoplasms inferred from phylogenies

Nicholas Williams et al.

Summary: By sequencing the clones of 12 patients with myeloproliferative neoplasms, we identified that early driver mutations are the foundation for tumor development and are linked to lifelong growth and evolution. This discovery provides opportunities for early intervention and a new model for cancer development.

The Response to Oxidative Damage Correlates with Driver Mutations and Clinical Outcome in Patients with Myelofibrosis

Elena Genovese et al.

Summary: Myelofibrosis is a type of myeloproliferative neoplasm with poor prognosis and no response to traditional therapies. Driver mutations in JAK2 and CALR impact ROS production and oxidative DNA damage, with CALR-mutated patients showing higher ROS levels and lower antioxidant activity compared to JAK2-mutated patients. High plasma levels of total antioxidant capacity are associated with adverse clinical features and poor overall survival, particularly in patients with JAK2 mutations, suggesting a different response to oxidative stress as a potential mechanism underlying myelofibrosis progression.

The longitudinal dynamics and natural history of clonal haematopoiesis

Margarete A. Fabre et al.

Summary: Clonal haematopoiesis, a common phenomenon in human tissues during aging, was studied by tracking 697 clones from 385 individuals aged 55 years or older over a median of 13 years. The study found that 92.4% of the clones expanded at a stable exponential rate, with different mutations driving different growth rates. Clones with the same mutation had growth rates differing by approximately +/- 5% per year. The study also revealed that mutations driving faster clonal growth carried a higher risk of malignant progression.

Primary myelofibrosis: 2021 update on diagnosis, risk-stratification and management

Ayalew Tefferi

Summary: Primary myelofibrosis (PMF) is a myeloproliferative neoplasm characterized by stem cell-derived clonal myeloproliferation, with common features including bone marrow fibrosis, abnormal inflammatory cytokine expression, anemia, hepatosplenomegaly, etc. Diagnosis is primarily based on bone marrow morphology. Treatment strategies can vary based on risk stratification, with options such as observation, drug therapy, or hematopoietic stem cell transplant.

Summary: This study identified two distinct pro-fibrotic mesenchymal stromal cell subsets in murine models and patient samples, showing their functional reprogramming at different disease stages. Additionally, the expression of the alarmin complex S100A8/S100A9 marked disease progression towards the fibrotic phase in both murine models and patient samples.

Oncometabolite L-2-hydroxyglurate directly induces vasculogenic mimicry through PHLDB2 in renal cell carcinoma

Huan Wang et al.

Summary: Metabolism reprograming is a hallmark of cancer, and the aberrant metabolism in renal cell carcinoma leads to the accumulation of L-2-hydroxyglurate, which promotes vasculogenic mimicry in renal cancer cells by reducing the expression of PHLDB2 through the activation of the ERK1/2 pathway. These findings suggest a potential therapeutic target for RCC.

Mutated clones driving leukemic transformation are already detectable at the single-cell level in CD34-positive cells in the chronic phase of primary myelofibrosis

Sandra Parenti et al.

Summary: The study utilized single-cell analysis to reveal the increased genomic complexity during disease progression of myeloproliferative neoplasms and the immunomodulatory effects of the treatment drug. The results indicated that the mutation order of different genes was related to the disease evolution process, and Ruxolitinib might induce immunosuppressive effects, ultimately leading to AML transformation.

Gene expression profile correlates with molecular and clinical features in patients with myelofibrosis

Sebastiano Rontauroli et al.

Summary: This study demonstrates that gene expression profiles in MF patients correlate with their survival and clinical features, particularly high-risk patients showing inferior overall survival and leukemia-free survival. High-risk patients are enriched in post-PV and post-ET MF, JAK2V617F homozygous patients, and typically display high molecular risk mutations.

Molecular pathogenesis of the myeloproliferative neoplasms

Graeme Greenfield et al.

Summary: Philadelphia negative myeloproliferative neoplasms (MPN) constitute a heterogeneous group of clonal myeloid stem cell disorders, including polycythaemia vera, essential thrombocythaemia and primary myelofibrosis. These disorders are characterized by constitutive activation of the JAK/STAT signaling pathway, often with driving mutations in JAK2, CALR and MPL. Co-occurring epigenetic factors may also play a role, while prognostic implications vary depending on disease phenotype, genotype and other external factors.
