New targets for treating hypertriglyceridemia

Klaus G. Parhofer

Summary: This article discusses the association between hypertriglyceridemia and atherosclerotic cardiovascular disease, as well as the mechanisms involved. It is found that cholesterol and apolipoproteins B and -CIII in triglyceride-rich lipoproteins directly induce and enhance atherosclerosis. Additionally, the concentration of triglyceride-rich lipoproteins affects the concentration, composition, function, and metabolism of LDL and HDL, increasing the risk. Therefore, apolipoprotein B and non-HDL cholesterol are more accurate predictors of risk in patients with hypertriglyceridemia. The current guidelines propose secondary goals related to non-HDL cholesterol and apolipoprotein B.

Atherogenic Plasma Index or Non-High-Density Lipoproteins as Markers Best Reflecting Age-Related High Concentrations of Small Dense Low-Density Lipoproteins

Sylwia Placzkowska et al.

Summary: The study aimed to assess the relationship between sdLDL and other metabolic disorder parameters and identify markers of increased sdLDL concentration. The results showed that different lipid parameters and atherogenicity indexes are associated with high sdLDL concentration depending on the age group. Abnormal API and non-HDL-C may indicate cardiovascular disease risk factors.

Insulin resistance and cardiometabolic indexes: comparison of concordance in working-age subjects with overweight and obesity

Summary: The aim of this study was to evaluate insulin resistance and cardiometabolic risk indexes in a population of overweight or obese workers. The results showed higher prevalence of insulin resistance and cardiometabolic risk among males, and varying percentages of workers identified as insulin resistant or at higher cardiometabolic risk depending on the index used. It was concluded that a combination of biomarkers and anthropometric measures can be used to calculate different indexes for personalized evaluation of insulin resistance and cardiometabolic risk.

Summary: Atherosclerosis is a chronic inflammatory disease driven by lipid accumulation, especially low-density lipoprotein, and dysfunction of lipid homeostasis. Inflammation plays a key role in all phases of atherosclerosis, while lipid metabolism and inflammatory pathways interact closely with each other. Studies have shown that microRNAs and lncRNAs also have important regulatory roles in the pathophysiology of atherosclerosis.

Summary: Recent studies focus on the role of triglyceride-rich lipoproteins in the development of atherosclerotic cardiovascular disease and the ongoing efforts to reduce cardiovascular risk by lowering TRL levels. Lifestyle changes remain key in prevention and treatment guidelines, with studies supporting the use of fibrates and niacin in specific cases. New targets like apolipoprotein C3 and angiopoietin-like protein-3 are driving the development of novel treatments for ASCVD risk reduction.
