The Design of Multi-target Drugs to Treat Cardiovascular Diseases: Two (or more) Birds on One Stone

Lucas Caruso et al.

Summary: Cardiovascular diseases are the leading cause of death worldwide. The traditional approach of developing cardiovascular drugs targeting a single molecule overlooks the multifactorial nature of the diseases. The current treatment strategy involves the combination of multiple drugs, and the development of multitarget drugs has become a promising strategy.

Long-term cardiovascular outcomes of COVID-19

Yan Xie et al.

Summary: Individuals with COVID-19, even those who were not hospitalized during the acute phase of the infection, are at an increased long-term risk of various cardiovascular disorders. A study using national healthcare databases found that beyond the first 30 days after infection, individuals with COVID-19 have an elevated risk of developing cerebrovascular disorders, heart diseases, and other cardiovascular complications. The risk increases depending on the care setting during the acute phase.

Structural Advances in Voltage-Gated Sodium Channels

Daohua Jiang et al.

Summary: Voltage-gated sodium channels are crucial for the rapid rising-phase of action potentials, and their mutations can lead to various human diseases. Recent studies using cryo-EM structures have provided valuable insights into the mechanism of eukaryotic Na-V channels, offering templates for drug development.

Structural Basis for Pore Blockade of the Human Cardiac Sodium Channel Nav1.5 by the Antiarrhythmic Drug Quinidine**

Zhangqiang Li et al.

Summary: The study presents the cryo-EM structure of human Na(v)1.5 channel bound to the antiarrhythmic drug quinidine, showing how quinidine blocks the pore and induces rotation of a Tyr residue for its mechanism of action. Comparison with a truncated rat Na(v)1.5 in the presence of another antiarrhythmic drug, flecainide, reveals distinct binding poses within the pore domain for the two drugs. This work, along with previous studies, sheds light on the molecular basis of action for class I antiarrhythmic drugs.

Summary: The high-resolution structures of human Na(v)1.1 and Na(v)1.5 channels associated with epilepsy and cardiac disorders were reported. Comparative structural analysis revealed common clusters of disease mutations in both channels. These mutations may affect the structural integrity and electromechanical coupling of the channels.

Nobiletin ameliorates cardiac impairment and alleviates cardiac remodeling after acute myocardial infarction in rats via JNK regulation

Zumei Liu et al.

Summary: Nobiletin has been found to protect against acute myocardial infarction-induced cardiac function decline and myocardial remodeling. Medium and high doses of Nobiletin significantly improve survival rate and cardiac function, and reduce infarction area and cardiac fibrosis. JNK inhibition was shown to play an important role in Nobiletin-induced antiapoptotic effects in myocardial infarction. Medium dose of Nobiletin demonstrated the strongest effect in vivo.

Fibroblast Growth Factor 21 Ameliorates NaV1.5 and Kir2.1 Channel Dysregulation in Human AC16 Cardiomyocytes

Jiamin Li et al.

Summary: The study found that FGF21 levels were significantly increased on day 1 after MI but returned to normal by days 7-10. Animal models showed that FGF21 treatment reduced ischemic arrhythmia incidence and duration in rats post-MI, and delayed the onset of arrhythmias in guinea pigs. In vitro studies demonstrated that FGF21 administration shortened action potential duration in AC16 cells and ameliorated ion channel remodeling through the Na(V)1.5/Kir2.1 pathway, potentially reducing postinfarcted arrhythmias.
