Evaluation of Pathologic Prognostic Factors in Neuroendocrine Tumors of the Small Intestine

Summary: The study reviewed 162 patients with small bowel NETs and found that primary tumor size and lymphovascular invasion were associated with lymph node metastasis, while TDs and higher pT stage were associated with distant metastasis. A cutoff of >= 7 positive lymph nodes predicted distant metastasis, and the modified pN stage was a better predictor than the prototypical pN stage. Higher pNmod and pM were associated with disease progression, worse progression-free survival, and worse disease-specific survival.

The clinical relevance of proper classification and the tools available to pathologists for accurate classification are explained. The article also discusses the differential diagnoses and theranostic biomarkers for NENs, as well as the role of pathologists in identifying precursor lesions and applying molecular immunohistochemistry.

It was found that increasing tumor size was associated with a higher risk of nodal disease, while factors such as age, pT4 disease, and nodal disease were associated with elevated serum CgA levels. Small App-NETs (<5 mm) that do not invade the serosa or mesoappendix are likely to be benign and low-grade.

Summary: Esophageal neuroendocrine neoplasms (E-NENs) are rare and aggressive, with subtypes including E-NET, E-NEC, and MiNEN, each with different histological features and prognoses. Gastric neuroendocrine neoplasms (G-NENs) also show a wide spectrum of behavior, with G-NETs associated with hypergastrinemia having a better outcome than G-NECs and MiNENs.
