Transbronchial cryobiopsy in lung transplantation: risk, reward and relevance

Lung allograft transbronchial cryobiopsy for critical ventilated patients: a randomised trial

Summary: This study compared transbronchial lung biopsy with 1.9mm and 2.4mm cryoprobes for critically ill lung transplant recipients requiring mechanical ventilation. The results showed that the 2.4mm cryoprobe group had the best quality of lung samples, providing the most definitive diagnoses, especially for acute lung rejection and rejection severity. The samples collected with the 2.4mm cryoprobe were larger, had more adequate alveolar tissue, more vessels per procedure, and no significant crush artefacts. Moderate bleeding was observed in 23% of cases, but no severe bleeding occurred.

Summary: This guideline provides evidence-based clinical practice recommendations for the role of transbronchial lung cryobiopsy (TBLC) in obtaining tissue-based diagnosis in patients with undiagnosed interstitial lung diseases (ILD). TBLC provides important diagnostic information with lower diagnostic yield compared to surgical lung biopsy (SLB), at reduced serious adverse events and length of hospitalisation.

Summary: This study reports the experience of outpatient TBLC implementation in a tertiary referral ILD center from the largest investigated TBLC cohort in Scandinavia. The diagnostic yield and prevalence of complications obtained by TBLC in unclassifiable ILD patients support outpatient TBLC as a valuable and safe alternative for diagnosing ILD in well-selected patients. The learning curves for TBLC were acceptable under experienced bronchoscopists.
