Subcutaneous implantable cardioverter-defibrillators: long-term results of the EFFORTLESS study

Pier D. Lambiase et al.

Summary: This study reports the 5-year outcomes of EFFORTLESS registry patients with early generation subcutaneous implantable cardioverter-defibrillator (S-ICD) devices. The study found that the spontaneous shock efficacy remained consistently high over the 5-year period and very few patients required replacement with a transvenous device. Additionally, early arrhythmic episodes did not predict later complications.

Air entrapment as a potential cause of early subcutaneous implantable cardioverter defibrillator malfunction: a systematic review of the literature

Hussam Ali et al.

Summary: This systematic review analyzed the phenomenon of air entrapment (AE) as a potential cause of early inappropriate shocks (ISs) following subcutaneous implantable cardioverter defibrillator (S-ICD) implantation. The study included 54 patients with AE as a potential cause of S-ICD malfunction, with an overall incidence of 1.2%. Radiologic diagnosis of AE was confirmed in 28% of the entire study cohort. The management approach of non-invasive management, primarily device reprogramming, appeared to be effective in most patients.

Primary Results From the Understanding Outcomes With the S-ICD in Primary Prevention Patients With Low Ejection Fraction (UNTOUCHED) Trial

Michael R. Gold et al.

Summary: The UNTOUCHED trial evaluated the efficacy and safety of contemporary S-ICD devices in a more typical, contemporary ICD patient population, demonstrating high freedom from IAS and shock rates. The study found a 18-month freedom from IAS of 95.9% and all-cause shock-free rate meeting the prespecified performance goal.

Subcutaneous implantable cardioverter-defibrillator electrode fracture: Follow-up, troubleshooting, and evaluation

Danesh K. Kella et al.

Summary: This case report highlights an unexpected inappropriate shock due to a partial fracture of the S-ICD electrode, emphasizing the importance of awareness and systematic follow-up to prevent fatigue failure fractures. Recommendations for clinicians on systematic S-ICD follow-up and troubleshooting are discussed in the study.
