Risks and Benefits of Ultra-Lung-Protective Invasive Mechanical Ventilation Strategies with a Focus on Extracorporeal Support

Darryl Abrams et al.

Summary: Ultra-lung-protective ventilation is a strategy that aims to achieve lower airway pressures and tidal volumes. Some patients may benefit from this strategy, but its application is limited by respiratory acidosis and associated hematological complications. Only in very severe cases of acute respiratory distress syndrome, using extracorporeal membrane oxygenation to accomplish ultra-lung-protective ventilation may have a favorable risk-to-benefit profile.

Mechanisms of oxygenation responses to proning and recruitment in COVID-19 pneumonia

Sandra Rossi et al.

Summary: This study aimed to investigate the mechanisms of oxygenation response to proning and recruitment maneuvers in patients with COVID-19 pneumonia. The findings indicate that the balance between resolution of dorsal atelectasis and formation of ventral atelectasis may play a key role in determining oxygenation responses. Patients assessed during the third week showed higher levels of consolidated tissue, potentially leading to fibrotic-like changes in the lung over time.

Prone-Positioning for Severe Acute Respiratory Distress Syndrome Requiring Extracorporeal Membrane Oxygenation

Matthieu Petit et al.

Summary: This study analyzed the characteristics and outcomes of patients with severe acute respiratory distress syndrome who received prone-positioned extracorporeal membrane oxygenation treatment. The results showed that prone-positioning during extracorporeal membrane oxygenation was safe and effective, and was associated with a higher probability of surviving and being weaned-off ECMO at 90 days. Additionally, quantitative lung CT-scan performed before prone-positioning can help predict the improvement in static compliance after that procedure.

Nitric oxide: Clinical applications in critically ill patients

Simone Redaelli et al.

Summary: Inhaled nitric oxide (iNO) is a selective pulmonary vasodilator that has been approved by the FDA for treating persistent pulmonary hypertension in newborns. While it has shown promising effects in preclinical studies, there is a lack of high quality evidence for its use in clinical settings. Various clinical applications have been suggested, but more randomized clinical trials are needed to determine the specific therapeutic roles of iNO.

Inhaled nitric oxide: role in the pathophysiology of cardio-cerebrovascular and respiratory diseases

Davide Signori et al.

Summary: Nitric oxide (NO) plays a crucial role in human life, being involved in both organ physiology and pathophysiology. This review aims to elucidate the mechanisms of NO in the respiratory and cardio-cerebrovascular systems under healthy or dysfunctional endothelium. It covers the production and effects of endogenous NO, as well as the disturbances related to NO depletion and excess. The potential therapeutic role of exogenous NO and the importance of NO in various clinical conditions are also discussed.

Prone positioning during venovenous extracorporeal membrane oxygenation for acute respiratory distress syndrome: a pooled individual patient data analysis

Marco Giani et al.

Summary: This study evaluated the association between prone positioning during extracorporeal support and ICU mortality in patients with severe ARDS using pooled individual patient data analysis of European cohort studies. The findings suggest that the use of prone positioning during ECMO was not significantly associated with reduced ICU mortality in a large population of ARDS patients.

Diminishing Efficacy of Prone Positioning With Late Application in Evolving Lung Injury

Yi Xin et al.

Summary: The effects of prone positioning on lung aeration may be influenced by the stage of lung injury, with less reinflation in late-stage patients compared to early-stage patients. Additionally, prone positioning decreased aeration in the anterior lung regions. These findings suggest that the efficacy of prone positioning in improving lung aeration may be limited by the stage of lung injury.
