The state of complement in COVID-19

Behdad Afzali et al.

Summary: Hyperactivation of the complement system is implicated in the pathology of COVID-19, with potential therapeutic benefits through complement inhibition. Latest research progress suggests targeting complement components for treatment may be beneficial in mitigating excessive inflammation and thrombosis in severe COVID-19 cases.

Multicenter Clinicopathologic Correlation of Kidney Biopsies Performed in COVID-19 Patients Presenting With Acute Kidney Injury or Proteinuria

Shreeram Akilesh et al.

Summary: This multicenter evaluation revealed that even mildly symptomatic cases of COVID-19 were associated with acute kidney injury and/or heavy proteinuria, leading to some patients requiring dialysis. Acute tubular injury was the most common renal pathology observed, but uncommon findings such as collapsing glomerulopathy and endothelial injury were also detected. Most patients progressed to irreversible kidney injury and dialysis.

Severe Coronavirus Disease 2019 (COVID-19) is Associated With Elevated Serum Immunoglobulin (Ig) A and Antiphospholipid IgA Antibodies

Omar Hasan Ali et al.

Summary: This study found that severe COVID-19 patients have a vigorous total IgA response and higher levels of IgA antibodies, which may be associated with complications of severe illness. Additionally, severe illness in COVID-19 patients is significantly correlated with elevated levels of anticardiolipin antibodies, suggesting a potential link to systemic autoimmunity.

COVID-19-Associated Glomerular Disease

Aneesha A. Shetty et al.

Summary: This study identified six recent African ancestry patients who developed COVID-19-associated AKI with podocytopathy, collapsing glomerulopathy, or both. Genetic testing in three patients confirmed high-risk APOL1 genotypes, suggesting a potential link between glomerular disease in COVID-19 patients and high-risk APOL1 genotypes.

Rebecca M. May et al.

Summary: This study conducted kidney biopsies on 284 COVID-19 patients in an international collaboration, identifying diseases that have increased in COVID-19 patients and demonstrating a high frequency of high-risk APOL1 genotypes within this group. No direct SARS-CoV-2 viral infection was found within the kidney.

Matthieu Legrand et al.

Summary: Acute kidney injury (AKI) is a common complication of COVID-19, and potential mechanisms for COVID-19-associated AKI include systemic haemodynamic instability, local immune cell infiltration, and endothelial injury, which may lead to impaired kidney function.

Impact of the COVID-19 Pandemic on Kidney Diseases Requiring Renal Biopsy: A Single Center Observational Study

Samy Hakroush et al.

Summary: The COVID-19 pandemic resulted in delayed diagnosis of kidney diseases and an increased rate of renal biopsies in the postlockdown phase, particularly affecting patients admitted to the normal medical ward. Additionally, there was a shift towards more patients with hemoglobinuria during the pandemic, specifically in a subgroup with hypertensive nephropathy requiring renal biopsy.
