RET Fusion-Positive Papillary Thyroid Cancers are Associated with a More Aggressive Phenotype

Timothy M. Ullmann et al.

Summary: This study aimed to investigate the different phenotypic tumor behavior in papillary thyroid cancer (PTC) driven by different genetic drivers. The results showed that RET-driven tumors were more aggressive than tumors with BRAF(V600E) mutation or RAS mutation, with higher rates of extrathyroidal extension, multifocal disease, and distant metastases.

Summary: The impact of multifocality on the prognosis after RFA for low-risk PTMC was minimal, indicating that RFA may be a promising treatment for both unifocal and multifocal PTMC in properly selected patients after sufficient preoperative evaluation.

Lin Yan et al.

Summary: This study evaluated the long-term efficacy and safety of radiofrequency ablation (RFA) for low-risk papillary thyroid microcarcinoma (PTMC) in a large population. The results showed that RFA was effective in treating PTMC with significant reduction in tumor volume, low incidence of complications, and no life-threatening events during the follow-up period.

Indications and Strategy for Active Surveillance of Adult Low-Risk Papillary Thyroid Microcarcinoma: Consensus Statements from the Japan Association of Endocrine Surgery Task Force on Management for Papillary Thyroid Microcarcinoma

Iwao Sugitani et al.

Summary: The management of patients with low-risk papillary thyroid microcarcinoma (PTMC; T1aN0M0) has become an important clinical issue, with the adoption of active surveillance (AS) to avoid overtreatment gradually increasing worldwide. However, not all PTMCs are suitable for AS, and the Japan Association of Endocrine Surgery has produced consensus statements regarding indications and strategies for AS in adult patients diagnosed with low-risk PTMC. These statements provide guidelines for AS and recommend immediate surgery for certain high-risk features in PTMC patients.

Current practice in patients with differentiated thyroid cancer

Martin Schlumberger et al.

Summary: Considerable changes have occurred in the management of differentiated thyroid cancer (DTC) over the past four decades, with improved diagnostic tools and prognostic classifications available. Low-risk patients make up the majority with good outcomes, while high-risk patients require more aggressive treatment and follow-up due to frequent recurrences.

Risk Factors for Central and Lateral Lymph Node Metastases in Patients With Papillary Thyroid Micro-Carcinoma: Retrospective Analysis on 484 Cases

Yijie Huang et al.

Summary: PTMC patients have a significant incidence of lymph node metastases. Risk factors for CLNM include age, gender, and tumor size, while extrathyroidal extension and number of CLNM are strong indicators for LLNM. Identifying these factors can help tailor individual surgical interventions for PTMC patients.

Summary: This study compared the clinical outcomes of radiofrequency ablation (RFA) versus thyroid lobectomy (TL) for low-risk papillary thyroid microcarcinoma (PTMC) and found comparable results in a four-year follow-up period. RFA, as a minimally invasive modality, may be a promising alternative treatment option for low-risk PTMC patients.

Long-Term Efficacy of Ultrasound-Guided Laser Ablation for Papillary Thyroid Microcarcinoma: Results of a 10-Year Retrospective Study

Ho Jin Kim et al.

Summary: This study evaluated the 10-year efficacy and safety of laser ablation (LA) for the treatment of solitary papillary thyroid microcarcinoma (PTMC). The results showed that LA was well tolerated by patients, with a disappearance rate of 100% in ablation areas after 12 months. Thyroid hormone and autoantibody levels did not significantly change post-treatment, and the majority of patients did not experience recurrence. For those with recurrence, surgery was performed successfully.

Summary: Ultrasound-guided percutaneous laser ablation is a safe and effective alternative clinical treatment for low-risk papillary thyroid microcarcinoma, achieving a 100% success rate for single ablation with minimal side effects and no serious complications reported. Ablated lesions showed significant reduction in volume over time, with 100% complete absorption by 24 months post-ablation in majority of cases.

Clinical Effects of Microwave Ablation in the Treatment of Low-Risk Papillary Thyroid Microcarcinomas and Related Histopathological Changes

Chenya Lu et al.

Summary: Ultrasound-guided percutaneous microwave ablation is a safe and effective treatment for low-risk papillary thyroid microcarcinoma, with significant reduction in tumor volume and some cases achieving complete remission. The most common histopathological changes after MWA include fibroblastic proliferation and chronic inflammation. Thyroid function may transiently decrease after MWA but returns to normal thereafter, and no serious complications were reported.
