Evaluation of Intima-Media Thickness and Arterial Stiffness as Early Ultrasound Biomarkers of Carotid Artery Atherosclerosis

Veronica Fernandez-Alvarez et al.

Summary: Carotid atherosclerosis is a major and potentially preventable cause of ischemic stroke, with early detection of subclinical atherosclerosis being crucial for initiating vascular prevention strategies. Carotid intima-media thickness (CIMT) and arterial stiffness are important new biomarkers in the early stages of carotid atherosclerosis, providing reliable markers for cardiovascular risk prediction, particularly in young individuals. Despite the controversy surrounding the usefulness of CIMT measurement in improving cardiovascular risk models, carotid stiffness has been shown to add value to risk prediction based on traditional factors like Framingham risk assessment, especially for those at intermediate cardiovascular risk.

Summary: This review summarizes the latest research on the epidemiology, genetics, clinical features, and management of spondyloarthritis (SpA) among Chinese populations. The prevalence of ankylosing spondylitis (AS) and HLA-B27 in AS patients in China is high, with HLA-B*2704 being the most common subtype. Tumor necrosis factor inhibitors and IL-17A inhibitors have shown effectiveness and safety for AS patients in China.

Cardiovascular Morbidity in Ankylosing Spondylitis: A Focus on Inflammatory Cardiac Disease

Pradnya Brijmohan Bhattad et al.

Summary: Ankylosing spondylitis is associated with an increased risk of cardiovascular disease, likely due to inflammation accelerating atherosclerosis and cardiac inflammation itself. However, establishing a clear link between AS and CV disease is challenging due to the rarity of AS.

Role of the IL-23/IL-17 Pathway in Rheumatic Diseases: An Overview

Claudia Schinocca et al.

Summary: IL-23 is a pro-inflammatory cytokine composed of two subunits and has a key role in protective immune response, but dysregulation can exacerbate chronic immune-mediated inflammation. Activation of the IL-23/IL-17 axis contributes to the development of various inflammatory diseases.

Subclinical atherosclerotic disease in ankylosing spondylitis and non-radiographic axial spondyloarthritis. A multicenter study on 806 patients

Inigo Gonzalez Mazon et al.

Summary: This study compared the burden of atherosclerosis between ankylosing spondylitis (AS) and nonradiographic axial spondyloarthritis (nr-axSpA), finding no differences in carotid plaques or cIMT between the two groups. A model containing age, BASFI, and ESR was able to identify high cardiovascular risk patients with high specificity.

Ankylosing spondylitis: an autoimmune or autoinflammatory disease?

Daniele Mauro et al.

Summary: Ankylosing spondylitis (AS) is a chronic inflammatory disease with features of both autoimmune and autoinflammatory processes. While the innate immune system plays a dominant role in AS, factors such as the intestine and biomechanical elements may also contribute to the disease's pathogenesis. Studies suggest a combination of autoinflammation and autoimmunity in AS, with evidence supporting both hypotheses.

The Role of the IL-23/IL-17 Axis in Disease Initiation in Spondyloarthritis: Lessons Learned From Animal Models

Mohamed Mandour et al.

Summary: Spondyloarthritis (SpA) is a chronic inflammatory joint disease where the IL-23/IL-17 pathway and TNF-α cytokine play important roles in its pathogenesis. Animal models have provided new insights into molecular mechanisms at different stages of SpA, aiding in understanding disease initiation and potential therapeutic strategies.

Summary: The study found that levels of Visfatin were elevated in patients with ankylosing spondylitis (AS) and were associated with impaired FMD and increased cIMT. Increased levels of Visfatin may be associated with subclinical atherosclerosis in AS.
