Summary: Intracoronary physiology testing is a valuable diagnostic approach for chronic coronary syndrome, but the simplifications made need to be counterbalanced against the increasing burden of physiological assumptions. Measurements of both coronary pressure and flow allow for comprehensive physiological evaluation of the vasculature, but widespread adoption has been hindered by perceived technical complexity and underappreciation of the role of the microvasculature. Meanwhile, non-invasive tools have matured, offering various techniques such as computational fluid dynamic and quantitative perfusion analysis.
