Resistance Phenotype and Molecular Epidemiology of Carbapenem-Resistant Klebsiella pneumoniae Isolated from Nanjing Children's Hospital in Jiangsu China

Summary: This study identified the drug resistance phenotype and molecular epidemiological characteristics of carbapenem-resistant Klebsiella pneumoniae (CRKP) strains among children in Jiangsu Province, China. The majority of the CRKP strains carried the blaKPC-2 carbapenemase gene, and a few strains carried other carbapenemase genes such as blaNDM-1, blaNDM-5, blaIMP-8, and blaOXA-181. Clonal dissemination of KPC-2-producing CRKP ST11 was observed in different departments, while non-ST11 strains showed high molecular typing diversity. It is the first report of NDM-5 and OXA-181-coproducing Klebsiella pneumoniae causing infection in children in China, posing a significant health risk to pediatric patients. Active surveillance and effective control measures are urgently needed to prevent further transmission of these strains among children.

Summary: This study found that central line-associated bloodstream infections were more common in solid tumor patients than in hematological malignancy patients, with a higher detection rate of gram-negative bacteria. Escherichia coli was the most common gram-negative organism, while coagulase-negative staphylococci were the most common gram-positive bacteria. The incidence of multi-drug resistance gram-negative bloodstream infections increased annually, while the mortality rate remained unchanged around 20% for gram-negative BSIs, but significantly higher at 35.4% for resistant gram-positive BSIs.
