The results showed that elevations in cTnT concentrations in patients with active chronic SMDs are common and not attributable to cardiac disease, while this phenomenon was not observed for cTnI. The re-expression of cTnT in skeletal muscle may partially explain this.

Summary: Serial measurements of cardiac troponin have been recommended by international guidelines for diagnosing myocardial infarction since 2000. However, differences exist in the guidelines published between 2020 and 2021 regarding the management of chest pain patients without ST-segment elevation, especially in terms of diagnosing NSTEMI. The Study Group of Cardiac Biomarkers in Italy decided to revise their document and provide suggestions for the use of biomarkers in clinical practice, focusing on the Italian setting.

High-sensitive cardiac troponin for the diagnosis of acute myocardial infarction different chronic kidney disease stages

Summary: This study aimed to determine the optimal cutoff-values of hs-cTn levels in patients with chronic kidney disease (CKD) and suspected acute myocardial infarction (AMI). It was found that the optimal value of hs-TnI in diagnosing AMI was 1.15 ng mL(-1), which was higher in males than females and increased monotonically with decreasing of estimated glomerular filtration rate (eGFR). Future multicenter randomized controlled clinical studies are needed to validate these findings.

Genome-wide association study of cardiac troponin I in the general population

Mendelian randomization analysis confirmed the non-causal role of cTnI in acute myocardial infarction but found indications for a causal role in heart failure, although not supported by all analyses.
