Prognostic Value of Mid-Region Proadrenomedullin and In Vitro Interferon Gamma Production for In-Hospital Mortality in Patients with COVID-19 Pneumonia and Respiratory Failure: An Observational Prospective Study

Summary: Coagulopathy and immune dysregulation have been identified as important causes of adverse outcomes in COVID-19. This study found that mid-region proadrenomedullin (MR-proADM) is associated with endothelial damage and has been proposed as a prognostic factor for COVID-19. In vitro interferon gamma (IFN gamma) production correlates with infection risk and mortality. MR-proADM at admission and during hospitalization is strongly associated with in-hospital mortality in COVID-19 pneumonia patients. Low in vitro IFN gamma production after the first week of hospitalization is linked to mortality in non-vaccinated patients, possibly identifying those with higher degrees of immune suppression.

Summary: This study found that MR-proADM has the highest predictive value for in-hospital mortality in patients with COVID-19 compared to other indicators. Non-survivors had older age, lower hemoglobin oxygen saturation, lower lymphocyte levels, higher lactate dehydrogenase and aspartate aminotransferase levels, and higher MR-proADM levels.
