Prevalence of preoperative cognitive impairment in older surgical patients.: A systematic review and meta-analysis

Paras Kapoor et al.

Summary: The prevalence of preoperative cognitive impairment in older surgical patients is not well-established. This systematic review and meta-analysis found that the pooled prevalence of unrecognized cognitive impairment was 37.0% in elective non-cardiac surgeries and 50.0% in emergency surgeries. Preoperative cognitive screening is crucial for risk assessment and stratification.

Perioperative Brain Health in the Older Adult: A Patient Safety Imperative

Susana Vacas et al.

Summary: While older adults account for 16% of the population in the US, they undergo over 40% of surgical procedures. Maintaining brain health after anesthesia and surgery is crucial, as aging decreases resilience to perioperative stress and often leads to undiagnosed cognitive impairment. This review summarizes the current understanding of perioperative neurocognitive disorders and recommends age-focused interventions to mitigate risk and improve outcomes for older patients. The potential impact of sleep and cognitive health, as well as future research directions, are also discussed.

Postoperative outcomes in older surgical patients with preoperative cognitive impairment: A systematic review and meta-analysis

Lina Chen et al.

Summary: This study aimed to investigate the impact of cognitive impairment and dementia on adverse outcomes in older surgical patients. The findings showed that preoperative cognitive impairment significantly increased the risk of delirium, mortality, discharge to assisted care, 30-day readmissions, and postoperative complications. Dementia was also found to increase the risk of 1-year mortality.

Efficacy of Intraoperative Hemodynamic Optimization Using FloTrac/EV1000 Platform for Early Goal-Directed Therapy to Improve Postoperative Outcomes in Patients Undergoing Coronary Artery Bypass Graft with Cardiopulmonary Bypass: A Randomized Controlled Trial

Sirirat Tribuddharat et al.

Summary: The study showed that using the FloTrac/EV1000 platform for early goal-directed therapy (EGDT) can significantly reduce the intensive care unit stay, mechanical ventilation time, and hospital length of stay in patients undergoing cardiac surgery.

Outcomes of transcatheter aortic valve replacement in patients with cognitive dysfunction

Vardhmaan Jain et al.

Summary: This study found that patients with dementia undergoing TAVR had a longer hospital stay as well as higher rates of discharge to a rehabilitation facility and in-hospital delirium, indicating potential debility and functional decline during hospitalization. However, in-hospital mortality and other outcomes were comparable between patients with and without dementia. Prospective studies are needed to standardize cognitive scoring and evaluate post-procedural quality of life.

Impact of delirium in acute cardiac care unit after transcatheter aortic valve replacement

Tania Luque et al.

Summary: In patients admitted to ACCU after TAVR, delirium is a common complication which is associated with factors such as prior cognitive impairment, peripheral arterial disease, use of general anaesthesia, and prolonged mechanical ventilation. Patients with delirium have a longer hospital stay and higher mortality rates.

Effects of comprehensive geriatric care models on postoperative outcomes in geriatric surgical patients: a systematic review and meta-analysis

Aparna Saripella et al.

Summary: This systematic review and meta-analysis found that comprehensive geriatric care models did not significantly improve clinical outcomes, including the prevalence of delirium, length of hospital stay, 30-day readmission rates, and 30-day mortality in elderly surgical patients. Further randomized controlled trials are needed to evaluate the effectiveness of these models on postoperative outcomes.

Neuroinflammation in cognitive decline post-cardiac surgery (the FOCUS study): an observational study protocol

Annemieke M. Peters van Ton et al.

Summary: This study aims to assess neuroinflammation in patients undergoing major cardiac surgery and investigate its relationship to cognitive outcomes, changes in brain structure and function, and systemic inflammation. The researchers will use neuroimaging techniques, MRIs, and neurocognitive assessments to gather data and analyze the inflammatory response in the brain. The results of this study will be published in international medical journals and presented at medical conferences.
