Impact of Anti-amyloid-β Monoclonal Antibodies on the Pathology and Clinical Profile of Alzheimer's Disease: A Focus on Aducanumab and Lecanemab

Mingchao Shi et al.

Summary: Alzheimer's disease is the most prevalent form of age-related dementia in the world, with limited treatment options available to provide only partial symptomatic relief. Disease-modifying therapies, targeting the systemic failure of cell-mediated Aβ clearance, are currently being studied in clinical trials.

Effect of reductions in amyloid levels on cognitive change in randomized trials: instrumental variable meta-analysis

Summary: Pooled evidence from 14 randomized controlled trials targeting amyloid mechanism suggests that reducing amyloid levels does not substantially improve cognition.

To what degree is late life cognitive decline driven by age-related neuropathologies?

Patricia A. Boyle et al.

Summary: The study found that cognitive decline in old age is driven by a wide array of neuropathologies, with Alzheimer's disease, infarcts, non-Alzheimer's disease neurodegenerative diseases, and cerebrovascular conditions playing significant roles. While most pathological indices were associated with faster decline, they only accounted for a portion of the variation in decline, highlighting the complexity of cognitive ageing.

Regulation of beta-amyloid production in neurons by cholesterol

Hao Wang et al.

Summary: The study shows that apoE uses astrocyte-derived cholesterol to traffic neuronal APP in and out of lipid clusters, generating A beta peptide. Deletion of astrocyte cholesterol synthesis reduces amyloid and tau burden in a mouse model of AD, indicating that cholesterol is kept low in neurons and astrocytes regulate A beta accumulation through cholesterol signaling.

The development and convergence of co-pathologies in Alzheimer's disease

John L. Robinson et al.

Summary: This study found that CAA, LATE-NC, and Lewy bodies are common pathologies in Alzheimer's disease that worsen in severity as the disease progresses. Age was associated with all pathologies, while the APOE epsilon 4 gene was specifically linked to CAA. In individuals with dementia, age was linked to LATE-NC, while Lewy bodies were more common in younger patients.

Failure to demonstrate efficacy of aducanumab: An analysis of the EMERGE and ENGAGE trials as reported by Biogen, December 2019

David S. Knopman et al.

Summary: Aducanumab recently underwent two large phase III clinical trials that were stopped prematurely by the sponsor due to divergent outcomes. One trial showed potential benefits from high doses of Aducanumab, while the other did not show any benefits. Further analysis is needed to determine the true efficacy as a treatment for Alzheimer's disease.
