Elevated pulse pressure is linked to cerebrovascular pathology and white matter damage, with associations between pulse pressure and neural integrity differing by sex and age. In this study, higher pulse pressure correlated with lower restricted isotropic diffusion in white matter, particularly in certain brain regions, with stronger effects in younger participants and in women. Monitoring microstructural changes using restriction spectrum imaging could indicate neuronal loss, demyelination, or inflammation associated with age-related cerebrovascular dysfunction.
