The 2021 WHO Classification of Tumors of the Pleura: Advances Since the 2015 Classification

Summary: The 2021 WHO Classification of Tumors of the Pleura and Pericardium has made substantial changes compared to the 2015 classification. These changes include the merging of pleural and pericardial tumors, renaming of certain tumor types, elimination of certain terms, addition of new classification categories, incorporation of new prognostic features, introduction of grading system, and identification of useful immunohistochemical markers.

Malignant Peritoneal Mesothelioma Associated With Endometriosis: A Clinicopathologic Study of 15 Cases

Summary: Malignant peritoneal mesothelioma (MPeM) associated with endometriosis is rare, and this study aimed to explore factors contributing to its pathogenesis. Clinicopathologic characteristics of 15 cases of MPeM associated with endometriosis were assessed, revealing a high prevalence of personal/familial history of malignancies in these patients.

The majority of PPM patients were male and Caucasian, and the prognosis was worse for patients with poorly differentiated tumors, tumor size > 4 cm, Caucasian race, and distant spread of disease.

Molecular Characterization of Peritoneal Mesotheliomas

BAP1 alteration/loss is associated with shorter survival, and a minority of specimens had genomic near-haploidization (GNH) associated with specific mutations. Localized Malignant Peritoneal Mesothelioma (LMPeM) in Women: A Clinicopathologic Study of 18 Cases

Summary: Localized malignant peritoneal mesothelioma is a rare tumor with limited information available. This study presents the experience with 18 cases seen in a hospital over a period of 43 years. Most patients were Caucasians, and the main symptom was abdominal pain. Microscopic examination revealed typical malignant mesothelioma features in most cases, but some cases had mixed features. Some patients had a history of asbestos exposure, and a family history of tumors was common. The majority of patients underwent surgery, and some also received adjuvant chemotherapy. The mean overall survival was 80.4 months, with a 3-year survival rate of 79% and a 5-year survival rate of 52.6%. Limited sample data suggest that small tumor size, low-grade cytology, and low mitotic index may be associated with a benign behavior.

Malignant Mesothelioma of the Peritoneum in Women A Clinicopathologic Study of 164 Cases

The findings include age distribution, symptoms presentation, immunohistochemical results, and treatment approaches. Results show that patients commonly present with abdominal or pelvic pain, and treatments primarily consist of chemotherapy, surgery, and hyperthermic intraperitoneal chemotherapy. Multivariate analysis identifies factors and treatments associated with overall survival rates.

Summary: PD-L1 expression in patients with malignant peritoneal mesothelioma is associated with germline and somatic mutations. Patients who had received prior chemotherapy showed varying levels of PD-L1 expression, with some exhibiting upregulation and others downregulation. The heterogeneity in PD-L1 expression before and after cytotoxic therapies may impact the effectiveness of immune checkpoint inhibition in this challenging disease.

Summary: While malignant peritoneal mesothelioma historically had a poor prognosis, recent decades have seen substantial improvements in outcomes. This study examined 225 cases and found that factors such as age, surgical approach, and histotype were significantly associated with overall survival. Low composite nuclear grade was independently associated with longer overall and disease-free survival.

Pediatric Mesothelioma With ALK Fusions A Molecular and Pathologic Study of 5 Cases

Summary: Pediatric mesotheliomas are rare, and this study reports 5 cases of malignant mesothelioma in children, characterized by fusions involving the ALK gene. The cases showed classic morphologic and immunohistochemical features of malignant mesothelioma, with ALK gene rearrangements investigated through molecular methods. These ALK-rearranged mesotheliomas share similarities with other fusion-positive mesotheliomas, occurring in young patients and having a simple, translocation-driven genetic profile.

Summary: Adenomatoid tumors (ATs) and malignant mesotheliomas (MMs) have different genetic characteristics, with ATs having a higher expression rate of L1CAM, while MMs have a higher loss rate of MTAP and BAP1 expression. In addition, TRAF7 mutation is a key factor distinguishing the development of AT from MM, and most AT cases are not associated with immunosuppression.

Dysdiadochokinesia, Ataxia, and Anemia: A Sign of Intraluminal Malignant Mesothelioma?

Summary: An 87-year-old man presented with altered mental status and ataxia, and was found to have a neuron-restricted antibody in his cerebrospinal fluid, raising concern for a paraneoplastic syndrome. Further imaging revealed abdominal lymphadenopathy, and push enteroscopy confirmed a jejunal mass, ultimately diagnosed as malignant mesothelioma. Malignant mesothelioma is more common in men, limited to the small bowel, and paraneoplastic syndromes associated with it are rare and have a poor prognosis.
