High-dose steroids in high pain responders undergoing total knee arthroplasty: a randomised double-blind trial

Niklas Nielsen et al.

Summary: This study involved 88 patients undergoing TKA with a preoperative pain catastrophising score >20 or regular opioid use, comparing high-dose and intermediate-dose preoperative intravenous dexamethasone. The results showed that high-dose dexamethasone reduced moderate-to-severe pain 24 hours after TKA and improved recovery in high pain responders without apparent side-effects.

Pain management after total knee arthroplasty PROcedure SPEcific Postoperative Pain ManagemenT recommendations

Patricia M. Lavand'homme et al.

Summary: The study identifies an optimal analgesic regimen for unilateral primary TKA, recommending paracetamol and NSAIDs, combined with specific nerve block and local infiltration analgesia, along with a single dose of intravenous dexamethasone. Morphine may be considered for hospitalized patients in rare situations. Opioids should be used as rescue analgesics postoperatively, while other analgesic interventions are not recommended. Future research on enhanced recovery programs and specific patient groups is needed.

A systematic review of pain outcomes reported by randomised trials of hip and knee arthroplasty

S. Koyuncu et al.

Summary: It is challenging to consolidate outcomes from randomized clinical trials that cover a variety of results. A systematic review of published outcomes after total hip or knee arthroplasty identified 165 outcomes categorized into six domains, with pain, analgesic consumption, and adverse events being the most reported. A primary outcome was reported in 50% of trials, with the consumption of rescue analgesics and pain being the most common.

Perioperative Systemic Dexamethasone Reduces Length of Stay in Total Joint Arthroplasty: A Systematic Review and Meta-Analysis of Randomized Controlled Trials

Johnathan R. Lex et al.

Summary: Perioperative systemic corticosteroids can reduce hospital length of stay, alleviate postoperative pain, and lower the incidence of postoperative nausea and vomiting following unilateral total knee and hip arthroplasty. Higher doses of corticosteroids are associated with greater reduction in pain and nausea and repeat dosing may further shorten hospital stay, although they may increase blood glucose levels.

Defining success in regional anaesthesia

Summary: This review aims to identify markers of success in regional anaesthesia, categorise these into an objective framework, and rationalise suggestions on how measuring outcomes in regional anaesthesia can be used to develop the widespread performance of this evolving subspecialty.

PROSPECT guideline for total hip arthroplasty: a systematic review and procedure-specific postoperative pain management recommendations

M. Anger et al.

Summary: This systematic review identified specific interventions that can effectively improve postoperative pain management, including medications and nerve blocks. Surgical and anesthetic techniques have minimal impact on postoperative pain, suggesting the choice should be based on other criteria.

Summary: Analysis of 122 studies showed that the use of PNB in total hip and knee arthroplasties was associated with lower odds ratios for various complications, recommending its use for improved outcomes in these surgeries.

Summary: The study aimed to compare the effectiveness of FIB versus control or placebo in postoperative pain, analgesic consumption, and complications for patients undergoing primary THA. The results showed that FIB did not demonstrate superiority in these aspects compared to control or placebo in primary THA patients.
