Immune Checkpoint Inhibition in GBM Primed with Radiation by Engineered Extracellular Vesicles

Summary: The delivery of therapeutic agents to glioblastoma (GBM) is hindered by the blood-brain barrier and immune suppressive microenvironment. This study demonstrates that modifying the surface of extracellular vesicles (EVs) with a brain-tumor-targeting peptide and using bursts of radiation can enhance EV delivery to GBM. Furthermore, loading EVs with small interfering RNA against programmed cell death ligand-1 (PD-L1) reverses radiation-stimulated PD-L1 expression and recruits tumor-associated myeloid cells, leading to improved therapeutic effects. These findings provide a new EV-based strategy for GBM immune checkpoint therapy.
