Substantial early changes in bone and calcium metabolism among adult pharmacoresistant epilepsy patients on a modified Atkins diet

Ellen Molteberg et al.

Summary: The aim of this study was to investigate the impact of the modified Atkins diet (MAD) on bone and calcium metabolism. The study found that after 12 weeks of MAD treatment, there were significant reductions in parathyroid hormone (PTH), calcium, C-terminal telopeptide collagen type 1 (CTX-1), procollagen type 1 N-terminal propeptide (P1NP), 1,25-OH vitamin D, and leptin. There was a significant increase in 25-OH vitamin D. These changes were most pronounced among patients under 37 years old and those with a higher body mass index (>= 25.8 kg/m(2)). The changes in sex hormones were not significant. The study suggests that MAD treatment may have a negative effect on bone health, and a reduced level of leptin may be a triggering mechanism.

Complementary and Alternative Medicine (CAM) use in Children with Epilepsy

Zahra Zhu et al.

Summary: This study examined the use of complementary and alternative medicine (CAM) among pediatric epilepsy patients in rural Pennsylvania. The study found that 13% of parents and caregivers reported CAM use by their children, primarily cannabis-related products. Dissatisfaction with traditional antiseizure drugs was a common reason for initiating CAM use. The study highlights the importance of healthcare providers initiating discussions about CAM and working collaboratively with parents and caregivers to ensure safe usage of CAM among pediatric patients.

Neuromodulation for Refractory Epilepsy

Philippe Ryvlin et al.

Summary: Three neuromodulation therapies using implanted devices and electrodes have been approved for drug-resistant focal epilepsy treatment. They are mainly palliative and show increased efficacy over time, with about half of patients experiencing significant seizure reduction. However, high-level evidence is currently lacking to determine the best treatment pathway for difficult-to-treat epilepsy.

Effect of ketogenic diet for drug-resistant epilepsy on immunological cells

May Fouad Nassar et al.

Summary: The study found that KD has no effect on neutrophil and lymphocytic counts as well as the number of adaptive and immune cells, but it does cause a reduction in phagocytic index. Further research on the full immunological profile and function is necessary to ensure the safety and efficacy of this therapeutic approach in drug-resistant epilepsy.

Switching Host Metabolism as an Approach to Dampen SARS-CoV-2 Infection

Sameh Soliman et al.

Summary: COVID-19 pandemic has altered daily lives worldwide, prompting the development of therapeutics and prophylactics to combat the viral infection. Recent recommendations highlight dietary modifications as a key approach post-lockdown. Changing host metabolism to a fat-dependent state, incorporating intermittent fasting, and supplementing with MCTs like lauric acid may hinder viral replication, serving as a potential prophylactic and adjunct therapy.

Summary: MEHMO syndrome, caused by a hemizygous variant in the EIF2S3 gene, is associated with significant morbidity and mortality. Glucose dysregulation and beta-cell damage in the pancreas may be linked to the pathophysiology of the syndrome. The adverse effects of ketogenic diet therapy in these patients should be considered.

The effect of ketogenic diet on serum lipid concentrations in children with medication resistant epilepsy

Unsal Yilmaz et al.

Summary: In children with medication-resistant epilepsy undergoing long-term KD treatment, a high proportion of them experienced dyslipidemia. Total cholesterol and triglyceride concentrations significantly increased in the first month of treatment, remained elevated for 24 months, but then showed a downward trend towards normal levels. However, this increase was not observed in patients with pre-existing dyslipidemia.
