The impact of nebulized epoprostenol and iloprost on hemoglobin oxygen affinity: an ex vivo experiment

Simon Woyke et al.

Summary: Inhalational prostacyclins improve oxygenation by acting as strong vasodilators. The study found that these prostacyclins have a direct effect on hemoglobin oxygen affinity, with different effects observed in males and females, as well as females taking oral contraceptives.

Inhaled Pulmonary Vasodilators in COVID-19 Infection: A Systematic Review and Meta-Analysis

Waleed Khokher et al.

Summary: This meta-analysis evaluates the role of inhaled pulmonary vasodilators (IPVD) in COVID-19 patients. The results show that although IPVD may improve oxygenation, there is no significant benefit in terms of mortality compared to standard therapy. Randomized controlled trials are needed to validate these findings.

Comparison of Fixed-Dose Inhaled Epoprostenol and Inhaled Nitric Oxide for Acute Respiratory Distress Syndrome in Critically Ill Adults

Mitchell S. Buckley et al.

Summary: The study compared the clinical and economic impact of inhaled nitric oxide (iNO) and fixed-dose inhaled epoprostenol (iEPO) in adult ARDS patients. The results showed that both treatments had similar effects on oxygenation, ventilation parameters, and clinical outcomes, with significant cost-savings realized with iEPO use.

Summary: Inhalation of NO significantly improved oxygenation in COVID-19 ARDS patients. Severe ARDS patients had higher partial pressures of oxygen and reduced pulmonary vascular resistance after receiving iNO. Significant changes in pulmonary shunting were not observed.

Rajiv Sonti et al.

Summary: In 80 mechanically ventilated COVID-19 patients receiving inhaled epoprostenol, 50% showed clinically significant improvement in PaO2/FiO2 after treatment. The translation results suggest that patients who were prone and had lower PaO2/FiO2 at the start of treatment were more likely to respond positively.

Causes of death and comorbidities in hospitalized patients with COVID-19

Sefer Elezkurtaj et al.

Summary: The study conducted autopsies on 26 patients who died after SARS-CoV-2 infection and COVID-19, finding that septic shock and multi organ failure were the most common immediate causes of death, often due to suppurative pulmonary infection. Most patients had comorbidities such as hypertension, ischemic heart disease, and obesity. The majority of deaths were found to be directly related to COVID-19, with contributory implications of preexisting health conditions to the mechanism of death.

Nitric Oxide to Fight Viral Infections

Fabio Lisi et al.

Summary: COVID-19, caused by SARS-CoV-2, has had a significant global impact, with efforts being made to find effective treatments, such as nitric oxide, which has shown antiviral properties against coronaviruses. Despite its potential benefits, the complex role of NO in host immune responses to viral infections and challenges in its development for viral therapies are discussed, along with suggestions for further research.

Summary: This study conducted in Northern Jordan investigated the incubation period and viral clearance time of COVID-19 hospitalized cases. The median incubation period was 6 days and the median viral clearance time was 13 days. Symptomatic presentation and abnormal chest radiograph were predictors for a prolonged viral clearance time.

This dysfunction may be secondary to diffuse intravascular micro- and macro-thrombosis or direct myocardial damage.

Summary: This systematic review and meta-analysis suggest that personal protective and social measures like handwashing, mask wearing, and physical distancing are associated with reductions in the incidence of COVID-19. However, due to heterogeneity of the studies, meta-analysis for other interventions like quarantine, isolation, universal lockdowns, and closures was not possible, and further research is needed.

Inhaled prostacyclin analogues in COVID-19 associated acute respiratory distress syndrome: scientific rationale

Eka Prasetya Budi Mulia et al.

Summary: Prostacyclin analogues are considered beneficial for treating CARDS due to their ability to improve oxygenation and pulmonary artery pressure, increase nitric oxide production, and potentially have anti-inflammatory effects. The inhaled form may be the ideal route of administration, with two ongoing clinical trials evaluating its efficacy in severe COVID-19 patients.
