Coronary Access After Transcatheter Aortic Valve Replacement With Commissural Alignment: The ALIGN-ACCESS Study

Giuseppe Tarantini et al.

Summary: This study investigated the impact of commissural alignment on the feasibility of coronary access after transcatheter aortic valve replacement (TAVR). The results showed that commissural alignment improves the rate of selective coronary access, but aligned supra-annular transcatheter heart valves (THVs) have a higher risk of unfeasible/nonselective coronary access compared to SAPIEN 3 THVs.

Unplanned Percutaneous Coronary Revascularization After TAVR A Multicenter International Registry

Giulio G. Stefanini et al.

Summary: The study aimed to evaluate PCI incidence and causes following TAVR, showing a declining trend over time and a shift from acute to chronic coronary syndromes as predominant indications. Unplanned PCIs post-TAVR are frequently successful, with no significant differences between balloon-expandable and self-expandable bioprostheses.

Transcatheter Replacement of Transcatheter Versus Surgically Implanted Aortic Valve Bioprostheses

Summary: This study compared the efficacy of TAVR in failed transcatheter aortic valves (TAVs) versus surgical aortic valves (SAVs) and found that TAVR in TAV failures had higher procedural success with similar procedural safety and mortality compared to TAVR in SAV.

Computed tomography analysis of coronary ostia location following valve-in-valve transcatheter aortic valve replacement with the ACURATE neo valve: Implications for coronary access

Arif A. Khokhar et al.

Summary: Post-implantation CT scans were used to evaluate the geometrical relationship between coronary ostia and valve frame in patients undergoing ViV-TAVR with the ACURATE neo valve. The study found that coronary access following ACURATE neo implantation for ViV-TAVR could potentially be challenging in a significant proportion of patients, especially with consideration to implantation depth.

Transcatheter aortic valve implantation in degenerated surgical aortic valves

Giuseppe Tarantini et al.

Summary: Transcatheter aortic valve implantation (TAVI) within failed bioprosthetic surgical aortic valves (valve-in-valve TAVI) is a well-established procedure for high-risk patients needing repeat aortic valve intervention. Challenges in valve-in-valve treatment include malposition, prosthesis-patient mismatch, and coronary obstruction, highlighting the importance of optimal patient selection and preprocedural planning to minimize risks. This review article provides a comprehensive overview of key periprocedural considerations for valve-in-valve TAVI.

ST-Segment Elevation Myocardial Infarction Following Transcatheter Aortic Valve Replacement

Laurent Faroux et al.

Summary: The study aimed to investigate the clinical characteristics, management, and outcomes of ST-segment elevation myocardial infarction (STEMI) after transcatheter aortic valve replacement (TAVR). Results showed that patients with STEMI after TAVR had longer door-to-balloon times and a higher PCI failure rate, leading to poorer outcomes attributed to coronary access issues specific to the TAVR population.

Feasibility of Coronary Access in Patients With Acute Coronary Syndrome and Previous TAVR

Won-Keun Kim et al.

Summary: This study aimed to evaluate the feasibility of CA and PCI in acute settings among patients who have undergone TAVR. The study found that CA or PCI after TAVR in acute settings is usually successful, but selective coronary engagement may be more challenging in the presence of long SFPs. Among patients requiring PCI, prior diabetes, cardiogenic shock, and failed PCI were predictors of early mortality.

Patient-Specific Implantation Technique to Obtain Neo-Commissural Alignment With Self-Expanding Transcatheter Aortic Valves

Gintautas Bieliauskas et al.

Summary: This study investigates a patient-specific implantation technique during TAVR to achieve safe and reproducible neo-commissural alignment of self-expanding transcatheter heart valves. The results suggest that this technique is feasible and safe in achieving optimal commissural alignment during TAVR procedures.

Transcatheter Versus Surgical Aortic Valve Replacement in Young, Low-Risk Patients With Severe Aortic Stenosis

Charan Yerasi et al.

Summary: TAVR is approved for patients of all risk profiles and ages, but has not been systematically tested in young, low-risk patients. Unanswered questions remain regarding safety and effectiveness in specific patient populations, future treatments, and technical considerations for TAVR. The authors predict a shift towards less invasive treatments due to patient preferences.
