Summary: Nonalcoholic fatty liver disease (NAFLD) may be a risk factor for hepatocellular carcinoma (HCC), and this study used meta-analysis to analyze the incidence rates of HCC in NAFLD patients. The results showed that NAFLD patients with cirrhosis have a similar risk of developing HCC compared to cirrhosis patients with other causes. However, the incidence of HCC in nonalcoholic steatohepatitis or simple steatosis patients is lower.

It can have serious negative consequences including overtreatment, financial toxicity, and psychological harm. Overdiagnosis can occur due to inaccurate diagnostic criteria, early detection of malignant lesions, indolent tumors, and competing risks of mortality. Guideline recommendations can help mitigate the risk, but more research is needed to understand and reduce overdiagnosis.

Summary: In this systematic review and meta-analysis, HCC surveillance was found to be associated with improved early-stage detection, curative treatment receipt, and survival in patients with cirrhosis. However, there was significant heterogeneity in the pooled estimates, and there were fewer data quantifying potential surveillance-related harms. Available data suggest that HCC surveillance is of high value in patients with cirrhosis, although further studies evaluating benefits and harms are still needed.

Changing global epidemiology of liver cancer from 2010 to 2019: NASH is the fastest growing cause of liver cancer

Summary: Liver cancer epidemiology is changing due to factors such as increasing alcohol consumption, rising obesity rates, and advancements in hepatitis B and C treatment. The Global Burden of Disease study estimated global and regional trends in liver cancer burden, revealing a 25% increase in liver cancer deaths between 2010 and 2019. Non-alcoholic steatohepatitis (NASH) and alcohol were found to have the fastest growing death rates, while hepatitis B and C showed declines. Urgent measures are needed globally to address metabolic risk factors and slow the growing burden of NASH-associated liver cancer, particularly in the Americas.

Summary: The sensitivity of ultrasound-based surveillance for early detection of hepatocellular carcinoma (HCC) in patients with cirrhosis is suboptimal. This study compared the performance of a novel multitarget HCC blood test (Mt-HBT) with ultrasound with or without alpha-fetoprotein (AFP) for early HCC detection. The results showed that Mt-HBT had higher sensitivity than ultrasound alone and similar sensitivity to ultrasound with AFP, suggesting that Mt-HBT could be a comparable alternative for HCC surveillance in at-risk patients.
