ACG Clinical Guideline for the Diagnosis and Management of Gastroesophageal Reflux Disease

Philip O. Katz et al.

Summary: This article provides updated recommendations and practical guidance for the evaluation and management of GERD, including pharmacologic, lifestyle, surgical, and endoscopic management. The use of proton pump inhibitors (PPIs) has raised concerns about long-term safety and overprescribing. New data regarding surgical and endoscopic interventions have also emerged.

Summary: This article provides updates on the guidelines for idiopathic pulmonary fibrosis (IPF) and addresses the progression of pulmonary fibrosis in patients with interstitial lung diseases (ILDs) other than IPF. The recommendations are based on consensus and systematic reviews, aiming to provide evidence-based guidance for clinicians.

Loss of SP-A in the Lung Exacerbates Pulmonary Fibrosis

Kyunghwa Kim et al.

Summary: This study explored the role of surfactant protein A (SP-A) in experimental lung fibrosis induced by low or high dose of bleomycin. The results showed that SP-A deficiency promoted the development of fibrotic damage. In vitro experiments demonstrated that SP-A inhibited the upregulation of fibrotic markers. These findings suggest that SP-A has an anti-fibrotic role in the development of lung fibrosis.

Consensus Gene Co-Expression Network Analysis Identifies Novel Genes Associated with Severity of Fibrotic Lung Disease

Summary: This study analyzed lung transcriptomic data to identify gene modules associated with idiopathic pulmonary fibrosis (IPF) and identified potential biomarkers. Validation studies confirmed the correlation of CRABP2 expression with decline in lung function.

Impact of gastroesophageal reflux disease on idiopathic pulmonary fibrosis and lung transplant recipients

Chanakyaram A. Reddy et al.

Summary: Idiopathic pulmonary fibrosis (IPF) patients often have gastroesophageal reflux disease (GERD), and GER-related micro-aspiration appears to be an important risk factor for IPF pathogenesis and deterioration of transplanted lung function. Medical GERD therapy and antireflux surgery may improve lung function.

Antacid Medication and Antireflux Surgery in Patients with Idiopathic Pulmonary Fibrosis A Systematic Review and Meta-Analysis

Summary: There is currently insufficient evidence to support the use of antacid medication or antireflux surgery for improving respiratory outcomes in patients with IPF. Well-designed prospective studies with objective evaluation of GER are needed to determine the role of these interventions.

Effectiveness of Proton Pump Inhibitors in Idiopathic Pulmonary Fibrosis A Population-Based Cohort Study

Summary: This study found no association between the use of PPIs and all-cause mortality or respiratory-related hospitalization in patients with IPF, suggesting that PPIs may not be as beneficial in treating IPF as some studies have suggested.

Idiopathic pulmonary fibrosis and gastroesophageal reflux disease: A population-based, case-control study

Misbah Baqir et al.

Summary: The study found that patients with IPF were more likely to have GERD compared to controls with non-IPF ILD or no diagnosed lung disease. This suggests that GERD may be an important factor contributing to the development of lung fibrosis.

Evaluation of Correlations between Genetic Variants and High-Resolution Computed Tomography Patterns in Idiopathic Pulmonary Fibrosis

Elisa Baratella et al.

Summary: This study found significant differences in HRCT patterns at diagnosis between familial and sporadic IPF patients, with familial patients showing a slight increase in typical UIP pattern at two years. Genetic factors play a key role in IPF development, but further studies are needed to clarify their impact on clinical treatment decisions.

Pharmacological Interactions of Nintedanib and Pirfenidone in Patients with Idiopathic Pulmonary Fibrosis in Times of COVID-19 Pandemic

Jose M. Serra Lopez-Matencio et al.

Summary: The discovery of antifibrotic agents has advanced the therapeutic management of idiopathic pulmonary fibrosis (IPF). Nintedanib and pirfenidone are now the foundation of IPF therapy, with potential to enhance treatment efficacy when used together. However, limited information exists on drug-drug interactions between these two medications, especially in polymedicated patients.

Long-term outcomes of transoral incisionless fundoplication for gastro-esophageal reflux disease: systematic-review and meta-analysis

Sabrina Testoni et al.

Summary: Transoral incisionless fundoplication (TIF) appears to be a long-term safe therapeutic option for selected patients with gastro-esophageal reflux disease (GERD) who are not suitable for lifelong medication or surgery, intolerant to proton pump inhibitors (PPIs), or at increased surgical risk. Most patients reported satisfaction with TIF, which significantly reduced PPI use, improved quality of life, and normalized heartburn and regurgitation symptoms.
