Controlling the past, owning the present, and future: cholinergic modulation decreases semantic perseverations in a person with post-stroke aphasia

Marcelo L. Berthier et al.

Summary: This pilot study aimed to evaluate the effectiveness of a cholinergic enhancing drug combined with speech language therapy in reducing SPs and other non-perseverative semantic errors in a person with chronic post-stroke aphasia. The results showed that the combined therapy significantly reduced SPs and improved everyday communication with low doses of donepezil.

Dose effects in behavioural treatment of post-stroke aphasia: a systematic review and meta-analysis

Sam Harvey et al.

Summary: There is currently insufficient evidence to determine the effect of treatment dose on response in aphasia interventions. The severity and chronicity of aphasia may affect the relationship between dose and response. Further dedicated research is needed to explore this relationship systematically.

Ten key reasons for continuing research on pharmacotherapy for post-stroke aphasia

Marcelo L. Berthier

Summary: This review highlights the importance of continuing research in pharmacotherapy for post-stroke aphasia (PSA) and emphasizes the potential benefits of modulating neurotransmitter systems with pharmacological interventions. Further research is warranted to explore the role of drug treatment in improving non-verbal cognitive, neuropsychiatric, and quality of life impairments accompanying PSA.

Post-Stroke Working Memory Dysfunction: A Meta-Analysis and Systematic Review

Selma Lugtmeijer et al.

Summary: This review systematically investigates the severity and nature of post-stroke working memory deficits, finding significant impairments in both low-load and high-load working memory tasks among stroke patients. These deficits remain prominent in the chronic stage of stroke and are associated with lesions in a widespread fronto-parietal network. Overall, stroke patients show moderate impairments across all subsystems of working memory, indicating a global nature of the impairment post-stroke.

Impact of COVID-19 on the Speech and Language Therapy Profession and Their Patients

Katie Chadd et al.

Summary: The UK's response to the COVID-19 pandemic posed challenges to healthcare services, leading to changes in speech and language therapy (SLT) services. Referral rates to SLT services decreased during the pandemic, with providers adopting more flexible approaches such as tele-therapy. Despite disruptions in care, therapy outcomes remained stable, and SLTs played a significant role in the neurorehabilitation of COVID-19 patients, particularly those with dysphagia.

Telerehabilitation for people with aphasia: A systematic review and meta-analysis

Cacciante Luisa et al.

Summary: This study evaluated the effectiveness or non-inferiority of telerehabilitation compared to conventional face-to-face speech therapy for people with aphasia. Results showed that telerehabilitation and face-to-face treatment were comparable in terms of gains achieved in auditory comprehension, naming accuracy, Aphasia Quotient, generalization, and functional communication skills for aphasia patients. The conclusion was that telerehabilitation training for aphasia appears to be as effective as conventional face-to-face treatment, although evidence is still insufficient for guiding clinical decisions.

Effect of Stroke on Contralateral Functional Connectivity

Grigori Yourganov et al.

Summary: Using a large sample of 116 participants who had experienced stroke, this study systematically examined the relationship between brain damage and functional connectivity within the intact hemisphere. The findings suggest evidence of contralateral cortical network reorganization in response to brain damage, particularly in well-connected regions where connectivity increases with the degree of damage.

The ventral pathway of the human brain: A continuous association tract system

Cornelius Weiller et al.

Summary: The brain hemispheres can be divided into upper dorsal and lower ventral systems. The prefrontal cortex is considered the platform for the highest level of integration, and the ventral pathway is now believed to be a continuous range covering different sensory modalities. Recent anatomical studies have shown that the ventral stream tracts in the human brain form a continuous band of fibers, providing a new perspective on the integration of information across different domains.

Novel Advances to Post-Stroke Aphasia Pharmacology and Rehabilitation

Natalia Cichon et al.

Summary: This review focuses on treatment strategies for post-stroke aphasia, including speech language therapies, cognitive neurorehabilitation, and pharmacotherapy. Research suggests that using TMS and tDCS can safely modulate cortical excitability, aiding in speech and language recovery. In addition to pharmacological treatments, specific interventions such as constraint-induced aphasia therapy have been proposed.
