Monoallelic IFT140 pathogenic variants are an important cause of the autosomal dominant polycystic kidney-spectrum phenotype

Summary: This study identifies a link between the IFT140 gene and autosomal dominant polycystic kidney disease (ADPKD). The monoallelic variation of IFT140 is associated with mild PKD phenotype, while the biallelic variation is linked to a syndromic ciliopathy. The findings also suggest that the monoallelic IFT140 variation is relatively common among cystic kidney disease patients.

Summary: Autosomal dominant polycystic kidney disease is mainly caused by mutations in PKD1 and PKD2 genes, but may also be caused by mutations in other endoplasmic reticulum protein biosynthetic pathway genes or somatic mosaicism. Genetic testing aids in diagnosis and prognosis, but screening for PKD1 mutations is challenging, and conventional Sanger sequencing is limited in elucidating the causes of atypical polycystic kidney disease.

Multicenter Study of Long-Term Safety of Tolvaptan in Later-Stage Autosomal Dominant Polycystic Kidney Disease

Summary: The study demonstrated that patients with prior long-term exposure to tolvaptan were more likely to experience aquaretic adverse events and liver enzyme abnormalities compared to those who were not exposed to tolvaptan. The genetic landscape of polycystic kidney disease in Ireland

Ireland is a valuable area for genetic studies of PKD, with diagnostic rates of 71-83% in Irish patients using different variant classification guidelines. The study reveals a spectrum of Irish autosomal dominant PKD pathogenic variants and highlights issues with implementing ACMG guidelines for variant pathogenicity interpretation in clinical genetics.

A case of TSC2-PKD1 contiguous deletion syndrome: Clinical features and effective treatment for epilepsy

Summary: Polycystic kidney disease with Tuberous sclerosis is a rare condition characterized by severe polycystic kidney growth due to deletions of the TSC2-PKD1 gene. The first case of PKDTS found in infancy in China showed typical neurological and renal symptoms, which were effectively controlled with Rapamycin treatment. The patient did not experience any renal function damage, highlighting the importance of early diagnosis and treatment.

Patients with Protein-Truncating PKD1 Mutations and Mild ADPKD

Summary: Despite having the most severe mutation class, 18% of patients with protein-truncating PKD1 mutations had mild disease on the basis of clinical and imaging assessment. Most of these mildly affected patients with protein-truncating PKD1 mutations reported a positive family history of ADPKD in preceding generations and displayed significant intrafamilial disease variability.
